摘要:
Nursing as an evolving science entertains the usage of terminology that lacks clarity and the singular application of terms. The resulting semantic confusion leads to difficulty in operationalizing definitions for use in taxonomic efforts and in research. Examples of this confusion exist where the term nursing diagnosis is used as concept, category, process, and product. Four suggestions are made to alleviate these difficulties.

摘要:
A working definition of nursing diagnosis was adopted by the North American Nursing Diagnosis Association (NANDA) Biennial Business Meeting in March 1990. Because of the working nature of the definition, members and nonmembers of the Association were invited to judge the merits and faults of the definition and to recommend areas needing further debate, analysis, or modification. The purpose of this article is to apply principles and rules of definition to an analysis of the working definition of nursing diagnosis as adopted by the NANDA General Assembly. In presenting this analysis, concerns and questions regarding the conceptual, logical, and grammatical clarity of essential elements (e.g., clinical judgment; individual, family, or community responses; actual or potential health problems; and actual or potential life processes) in the definition are posed; and, where appropriate, suggestions for clarification are offered. Further debate, research, and clinical testing is requested in an attempt to improve NANDA's very important contribution to the development of nursing practice theory.

Problem orientation has dominated the nursing diagnosis movement since the early 1970s. The purpose of this article is to discuss how the new nursing diagnosis definition provides opportunity for a wellness perspective to be incorporated into the movement. Wellness is differentiated from illness and health, and a conceptual model is offered to clarify these concepts. Work is reviewed from the growing number of authors who have supported wellness nursing diagnoses and advocated their incorporation into the current taxonomic listing. Cases of ill–well and well–ill clients and examples of wellness nursing diagnoses are included lo emphasize the need for wellness diagnoses across the wide scope of nursing practice. 摘要:
Nursing diagnoses are described by definition as the basis for selection of interventions to achieve health outcomes. Processes used to link diagnostic judgments with desired outcomes and interventions can be holistic or reductionistic. A model of clinical inference is presented to illustrate the interrelated nature of judgments about diagnoses, desired outcomes, and interventions. Implications for development of diagnostic and taxonomic tools to guide holistic judgments are explored.

摘要:
Through the Omnibus Budget Reconciliation Act of 1989 the Agency for Health Care Policy and Research (AHCPR) was established as the successor to the National Center for Health Services Research and Health Care Technology Assessment (NCHSR). Under the auspices of the Office of the Forum for Quality and Effectiveness in Health Care, AHCPR is responsible for developing and updating guidelines to be used in the management of clinical conditions. This is being accomplished through AHCPR's Medical Treatment Effectiveness Program (MEDTEP). MEDTEP's purpose is to improve the effectiveness and appropriateness of health care services by determining the effects of health care practices, including nursing practices on patient outcomes. MEDTEP is focusing on four sets of activities: guideline development, dissemination of research findings, data base development, and effectiveness and outcomes research.
