The relationships of exercise adherence and aerobic fitness to a spectrum of biologic and psychologic benefits were examined in the context of a three-stage model of exercise training: recruitment, participation, and continuation. Ninety-five men consecutively admitted for uncomplicated acute myocardial infarction or for coronary artery bypass graft surgery were screened for exercise training based on their ability to exercise and the distance of their residence from the exercise site, yielding a final sample of 50. Patients were followed up for a period of one year posthospitalization. Fifty percent of the patients attempted training. Exercise adherence declined by 50% after the completion of training and stabilized at that level for the rest of the year. Six variables predicted adherence at two of the three stages of the program throughout the year, while six other variables predicted adherence at only one of the stages. Adherence was positively associated with improvement in mood state as well as with improvement in ischemic and dyspneic symptoms. Neither the extent of participation in exercise training (beyond six weeks of the 12-week program), nor the continuation of exercise activity after training was related significantly to improvement in fitness. Further, improvement in fitness was not significantly associated with improvement in benefits. The pattern of positive relationships between exercise adherence and benefits and null relationships between exercise aerobic fitness and benefits are discussed in terms of a possible psychologic mechanism through which exercise training may exert its positive effects.

A 45-year-old male cardiac transplant patient (one of 58 such subjects taking part in an ongoing pilot project involving Harefield Hospital, Middlesex, England, and the Toronto Rehabilitation Centre, Canada) was given a medically prescribed 11-month endurance-type (walking/jogging) rehabilitation exercise program in order to see if a cardiovascular training effort could be achieved.Initial and final cycle ergometer exercise testing showed a marked increase in the maximal power achieved on the cycle ergometer from 650 kpm/min to 1,200 kpm/min (84%), the highest measured oxygen consumption from 1.7 L/min to 2.6 L/min (53%), a reduction in the resting and the submaximal heart rates at all workloads, and an increase in the ventilatory anaerobic threshold from 1.4 L/min to 1.9 L/min (35%).In April 1985, the patient was entered officially in the Boston Marathon under the auspices of the Toronto Rehabilitation Centre and the American Medical Joggers Association. He finished the 42-kilometer course comfortably and without subsequent ill effects in under six hours, the first such patient, to our knowledge, to accomplish such a feat.Although the overall results of the pilot project will not be available until the end of 1985, it is felt that the performance of this individual implies a favorable potential for posttransplant exercise rehabilitation.

Type A behavior, which is often defined as extremes of aggressiveness, hostility, time urgency, and competitive achievement striving, has been linked to the development of coronary heart disease. A structured interview is currently one of the standard ways of assessing this behavior pattern. Though Type A behavior as assessed by this type of interview successfully predicted disease in a large-scale epidemiologic study, a close look at the literature reveals that there are some problems with this assessment tool. Several versions of the interview have been published, there is no one standard scoring system, interviewers must attend a relatively expensive workshop to be trained, and little is known about the relationship between interviewing style and interviewee response. Most important, however, are recent data that have raised doubts about the validity of the structured-interview-assessed Type A construct.

Myocardial infarction patients who regularly attended an exercise rehabilitation program for a minimum period of five months (experimental group) were compared with patients who dropped out of the same program shortly after entry and who remained physically inactive. The results of the British Printing Corporation Life-style questionnaire indicated that although there was no statistically significant difference in the life-style of the two groups before the myocardial infarction, the experimental group's overall postinfarction life-style was significantly healthier (distinctly with regard to the food, exercise, and care factors). Furthermore, the Profile of Mood States (POMS) questionnaire showed that the control group had a greater mood disturbance than the experimental group did at the start of the program. Both groups, however, maintained their mood profile over the experimental period. The present study suggests that regular participation in an exercise rehabilitation program is associated with a healthier postmyocardial infarction life-style and that those patients who are likely to drop out of the program may be prospectively identified by the psychologic characteristics they exhibit on entry into the program.
