摘要:
Fine needkle aspiration biosy of palpable and nonpalpable mammary lesions has become a common procedure. In addition to its well-known ability to establish or suggest the diagnosis of carchinoma, fine-needle aspication biopsy has been used to identify proliferative, presumably precancerous breast lesions. The authors present a case illustrating diagnostic criteria and challeges in this area.

摘要:
The following case illustrates pseudoinfilanation of fibrous stroma of the breast by duetal carcinoma in situ (DCIS) subsequent to fine-needle aspiration (FNA). Epithelial clusters in the fibrous stroma mimicked small foci of influating carcinoma. However, features compatible with FNA effect include the presence of collections of hemosiderin-ladenmacrophages in the surrounding stroma, luminal fibrin and blood clots in ducts with DCIS, and the similarity of the epithelial clusters in the stroma to those trapped within the fibrin and blood clots. The lack of a desmoplastic response precluded the diagnosis of inflinating carcinoma. Invasion was nor found in either the biopsy or the mastectomy specimen. Awareness of a previous FNA or other needing procedure may prevent an crroncous histologic diagnosis of inflinating carcinoma. Fine-needle aspiration or instrumentation effect should be considered whenever the histologic findings are not classic for invasive carcinoma.

摘要:
Collagenous spherulosis is an uncommon, usually incidental finding in breast tissue removed for a variety of reasons. A case of collegenous spherulosis, presenting as mammographic microcalcifications and diagnosed by stereotactic core needle biopsy, is used to address this relatively new diagnosis modality for the evaluation of non-palpable breast lesions. In this case, the microcalcifications were present exclusively within aollagenous spherulosis and no other abnormalities were obseved. With the increasing use of stereotactic core-needle biopsy it is important that pathologies become familier with this technique and its limitations, as well as the unusual lesions it may uncover.

摘要:
Infiltrating plcomorphic lobular careinoma is a recently recognized, aggressive variant of lobular carcinoma that has the invasive pattern of classical infiltrating lobular carcinoma but that also has large plcomorphic nuclei and abundant cosinophilic, slightly granular cytoplasm. The authors described an infiltrating pleomorphic lobular carcinoma accompanied by an extensivein situcarcinoma that involved lobular units primarily. Thein situcarcinoma was cytologically identical to the infiltrating carcinoma and was interpreted as thein situcounterpart of infiltrating pleomorphic lobular carcinoma.

摘要:
A case of neoimplant formation after removal of a grossly ruptured silicone gel—filled breast implant without associated capsulectomy is reported. The 310g, 7.5 X 7 X 7 cm neoimplant developed over a period of 4 months. When removed, it had an inteact, thick, caleified, fibrous wall that was filled with dark green nonsticky material shown microseopically to be blood, fibrin, and proteinacenus material. No silicone gel was evident in the cystie fluid cither grossly or microscopically. However, the fibrous wall of the neoimplant was a dense sear with foamy macrophages containing refractile but not birefringent material indicative of silicone gel.
