摘要:
Polymorphous low-grade adenocarcinoma (PLGA) is a recently described, relatively uncommon tumor that arises preferentially in minor salivary glands, favoring the palace. The tumor is characterized by a proliferation of uniform, cytologically bland cells and exhibits a broad range of growth patterns including tubular, solid, cribriform, and fascicular. A focal papillary growth pattern may also be observed. Tumors are infiltrative at their borders with a single-file pattern of invasion being most common. Intraneural and perineural invasion is also a frequent finding. The histologic differential diagnosis includes both benign and malignant tumors. Clinically, PLGA follows an indolent course. These tumors are best treated by complete but conservative excision, and there is no compelling evidence that postoperative radiation or chemotherapy is of benefit. Likewise, radical neck dissection is not of benefit unless metastatic cervical disease is present. Although recurrent or regional disease may develop, distant metastatic disease is distinctly uncommon.

摘要:
The describe a case of calcifying epithelial odontogenic tumor (CEOT), also known as Pindborg tumor, an odontogenic tumor with interesting and distinctive histologic features. Although benign, CEOT is locally aggressive. and incomplete surgical resection often leads to recurrences; consequently, recognition of CEOT by the pathologist is essential for optimal patient management. In must cages, familiarity with the histologic futures of CEOT should permit i correct pathologic diagnosis to be rendered with little difficulty, although there ate variants, and mimics of CEOT with which tine should be familiar.

摘要:
Ameloblastoma is the most common benign odontogenic neoplasm. Despite this fact, few surgical pathologists have more than a general familiarity with this tumor. Experience is often comprised of study sets or as preparation for the anatomic pathology portion of the American Board of Pathology Certifying examination. Such is not a criticism but a simple tact resulting from the low incidence of the disease in the general population. The histologic spectrum of ameloblastoma is surprisingly broad. This face implies the need for pathologists to recognize histologic variants of the tumor and includes recognition of preameloblastomatous changes in odontogenic cysts. The purpose of the this case review is to present the salient features of one of the least common histologic variants of ameloblastoma and to use the discussion as a forum to present the features of other histologic types. As a corollary, discussion of pitfalls in diagnosis will be bristly presented.

An typical case. the surgical specimen is From the mandible. The microscopic features include a cellular fibrous connective tissue containing mineralized tissue that in areas is trabecular bone, although other areas reveal dropler or psammomitoid-like material. There is no atypia noted, and the initial impression is that the lesion is benign. The diagnostic dilemma faced is that of benign fibro-osseous lesions, A partial list of entities encompassed by this term includes fibrous dysplasia, cherubism, ossifying and cementifying fibroma, Paget's disease, and cemento-osseous dyaplasia. There is considerable overlap in the histologic features of the above, with die exception of cherubism. In the others, similar, if not identical, histologic fields could be found in each. Therefore, the term fibro-osseaous. lesion is descriptive and generic, and in most cases histologic features alone are inadequate to make a definitive diagnosis. The diagnosis depends on additional clinical and radiographic data, gross pathology, and in Paget's disease serum alkaline phosphatase levels. It is also important to identify whether lesions are confined to the jaws and whether they are singular or multiple. The current report presents a case of cemento-osseous dysplasia and discusses its clinicopathologic spectrum.

denoid cystic carcinoma is a malignant neoplasm that can occur in many head and neck locations including the nasal cavity, paranasal sinuses, lacrimal gland, esophagus, and in both minor and major salivary gland tissue. Understanding this form of adenocarcinoma is especially important because of its growth characteristics, high rate of recurrence,and potential for distant metastasis. Adenoid cystic carcinoma can present as a unilobular mass of the salivary glands. The practicing clinician and pathologist play a dual role in the diagnosis and treatment of this malignancy. Depending on the size and location of the tumor, treatment options can range from limited surgical excision to radical surgery with radiation therapy. Failure to include a malignant neoplasm in the differential diagnosis when confronted with a palatal tumor can lead to delays in treatment Early diagnosis of adenoid cystic carcinoma of the salivary glands is critically important to increased treatment success rates. The purpose of this case discussion is to emphasize features common to intraoral adenoid cystic carcinoma and to highlight the need for early diagnosis and appropriate treatment.

esenchymal chondrosarcoma is a variant form of chondrosarcoma, which may be related pathogenetically to the Ewing's sarcoma/primitive neuroectodermal tumor group. The clinical and pathologic features of a typical case occuring in a common location, the mandible. are presented together with a review of the literature and a discussion of The differential diagnosis, nosology, and treatmeat of mesenchymal chondrosarcomas. it is important to recognize the potentially aggressive but unpredictable nature of this tumor conferred by its small cell component, the possibility of a benign or malignant-appearing cartilage component, and the potential for late loca1 recurrence and lymphatic or systemic metastases. The preferred treatment is. radical surgical excision. The value of radiation or chemotherapy alone or as an adjunct to radical surgery is difficult to assess because of the rarity of the tumor.
