摘要:
Most lesions of the thyroid are follicular. These range from changes in the follicles seen diffusely, as in hyperthyroidism associated with Graves disease, to those lesions that present as multiple or single nodules.The solitary nodules of the thyroid, which are encapsulated, can be classified as adenoma, carcinoma, or a follicular variant of papillary carcinoma. Its peculiar nuclear features define a follicular variant of papillary carcinoma, which allows for its diagnosis in both cytologic and histologic preparations. The diagnostic distinctions between follicular adenoma and carcinoma necessitate the examination and interpretation of well-prepared histologic sections. Various criteria for the diagnoses of these neoplasms and groups of lesions will be discussed in this article. The article will concentrate on that group of lesions which are truly follicular neoplasms: adenoma and carcinoma.

Successful endocrine surgery is greatly facilitated by the interaction between the pathologist and the surgeon. 摘要:
It would appear that the presence of ultrasonographically detected calcifications is one of a number of clinical features that may warrant further investigation of a thyroid nodule. Larger studies defining the radiologic and pathologic correlation of such cases are needed prior to suggesting more definitive management guidelines for thyroid nodules with calcifications.

Although most follicular-derived thyroid carcinomas are well differentiated and have an excellent prognosis, certain forms of thyroid carcinoma behave in a clinically more aggressive fashion. We present a case of a 27-year-old woman who presented with a palpable, 3.0-cm right thyroid nodule. Fine needle aspiration showed a cellular lesion with many crowded groups of small follicular cells with scant cytoplasm, clumped chromatin, moderate nuclear pleomorphism, and mitotic activity. Surgical resection of the thyroid gland revealed an insular carcinoma with classic nested architecture arising within a microfollicular thyroid neoplasm. Two years later, the patient developed multiple liver nodules detected by computed tomography, and fine needle aspiration showed metastatic insular carcinoma. Insular carcinoma, in particular, as well as certain subtypes of papillary carcinoma, such as the tall cell, columnar cell, and diffuse sclerosing variants, is considered to be an aggressive form of follicular-derived carcinoma. Its clinical behavior is between that of well differentiated thyroid carcinoma and of anaplastic carcinoma, the most highly aggressive follicular-derived malignancy arising within the thyroid gland. Cytologic and histologic recognition of these aggressive variants of thyroid carcinoma is important because the specific diagnosis may impact the clinical management of these patients.

Pathologic assessment of the parathyroid glands can be difficult. The authors present three cases of the most frequently encountered scenarios in parathyroid disease: adenoma, hyperplasia, and carcinoma. These cases illustrate the practical issues surrounding the diagnosis and management of primary hyperparathyroidism. Other neoplasms that confront the surgical pathologist include secondary and tertiary hyperparathyroidism, hypoparathyroidism, and parathyroid cysts.

A suspected parathyroid adenoma was seen on a sestamibi scan. A repeat scan following an episode of neck pain failed to show the abnormality. The surgical specimen showed infarction and hemorrhage, likely accounting for the imaging discrepancy.

The pathologic diagnosis of various thyroid lesions can be challenging in the everyday practice of pathology. The two lesions that comprise a major portion of difficult cases in thyroid pathology include the follicular variant of papillary thyroid carcinoma and papillary carcinoma arising in association with lymphocytic thyroiditis. Both these neoplasms are equally difficult to diagnose from cytology and surgical pathology specimens. This article will provide a broad review of pathologic diagnosis, differential diagnosis, and the role of special studies in the diagnosis of these lesions. In addition, a practical approach to the pathologic diagnosis of these lesions is given on the basis of recent literature and personal experiences.
