Reversal of P-glycoprotein-associated multidrug resistance (MDR1) has received much attention in recent years. A number of agents has been identified which are capable of overcoming MDR1 in experimental tumor models. The clinical effec- tiveness of such chemosensitizers (CS) has been limited to date. Nonetheless, there are data suggesting that the concept of MDR1 reversal might also function in cancer patients. It always has been a concern that inhibition of physiological P-glycoprotein function may increase chemotherapy toxicity. Recent data suggest that CS can inhibit biliary and/or renal elimination of cytotoxic agents, resulting in increased bone marrow toxicity. It seems likely that such phenomena will be more frequently observed in the future as more potent CS become available. In order to making progress in this field, future clinical studies of CS must strictly adhere to the usual standards of clinical drug development, i.e., starting with phase I and pharmacokine-tic studies and only based on these results proceeding to activity studies (phases II and III). Clinical MDR1 reversal is a complex task, and it is only through carefully conducted trials that significant progress in this area may be possible. For the time being, the clinical use of CS continues to be an experimental approach and thus should not be pursued outside the context of controlled studies.

Despite the increasing use of adjuvant therapy locoregional relapse and skin metastases remain a problem in the treatment of patients with breast cancer. Topical administration of miltefosine supplements the available local treatment modalities. Especially patients with superficial lesions – no deep skin or soft tissue infiltrations, node size not exceeding 1-2 cm – may achieve clinically relevant remissions in about 40%. After superficial exulceration a scab formation and reepithelization may be expected. An attempt at topical treatment should be indicated if surgical excision and radiotherapy are no longer applicable or in case such therapy cannot be provided due to logistic difficulties. Concomitant local therapy, in addition to a systemic treatment indicated by nonskin metastases, may be meaningful and necessary. Together with surgery and radiotherapy as the treatments of choice, topical treatment with miltefosine constitutes a new treatment option which is reasonably well tolerated and can be employed in ambulatory patients. During the course of the disease the therapeutic response to skin metastases may become a major problem for patients, and additive local treatment therefore may be to improve considerably the quality of life of the patients. Das neue Antiöstrogen Droloxifen (DROL) zeichnet sich, verglichen mit Tamoxifen (TAM) oder Toremifen (TOR), durch eine mehr als lOfach höhere Bindungsaffinität zum Östrogenrezeptor (ER) aus. In der folgenden Studie wurde untersucht, ob diese höhere Bindungsaffinität sich entsprechend auf die Hemmung des Zellwachstums oder der Proteinbiosynthese auswirkt. Wir verglichen die antiproliferative Wirkung der drei Antiöstrogene auf die ER-positive menschliche Tumorzellinie MCF-7 Ml und die ER-negative menschliche Tumorzellinie MDA-MB-231 (als Negativkon-trolle) unter serumfreien Bedingungen. Effekte auf die Protein-Neusynthese wurden anhand des 14C-Leucin-Einbaus untersucht, während Veränderungen des Gesamtproteingehalts mit Hilfe des Fluoreszenzfarbstoffs Sulforhodamin-101 am Flow-Cytometer quantifiziert wurden. Die IC50 für die Wachstumshemmung lag bei der Zellinie MCF-7 Ml für DROL unter 0,03 µM, während sie bei TAM oder TOR um eine Zehnerpotenz höher, bei etwa 0,3 µM, lag. DROL hemmte im ER-abhän-gigen Konzentrationsbereich von 0,03-1 µM den 14C-Leucin-Einbau überraschen-derweise genauso effektiv wie Cycloheximid, ein bekannter Inhibitor der Proteinbiosynthese, während TAM oder TOR in therapeutisch relevanten Konzentrationen keine Wirkungen zeigten. Die Messungen am Flow-Cytometer bestätigten diese Ergebnisse. Zusammenfassend läßt sich sagen, daß eine höhere ER-Bindungsaffinität bei den nichtsteroiden Antiöstrogenen mit einer starker antiproliferat

Background: Miltefosine solution has been used for the topical treatment of skin metastases from breast cancer. An overview analysis of data from clinical phase II trials has been performed to identify prognostic factors for efficacy and tolerability. Materials and Methods: Results from 287 patients treated with miltefosine solution in phase II trials have been included in an overview analysis. This involved an extramural review of data on efficacy. Prognostic factors for quality of response and time to progression were analyzed by a logit and a Weibull model, respectively. Results: Depth of infiltration was identified as the most relevant prognostic factor for response of skin lesions. A 39% response rate (complete, partial and minor responses) was achieved in patients having small nodular lesions (diameter of nodules up to 1 cm) and/or lymphangitic infiltration without deep subcutaneous tumor mass. In patients with lesions outside these limits, the rate of objective regressions was 13%. In case miltefosine solution was added on top of a systemic endocrine therapy which had failed to control the skin lesions, a response rate of 38% to the combination was observed (median time to progression of skin lesions 27 weeks). Pruritus was the most frequently reported local skin reaction (31%), followed by erythema (18%), and pain and burning (10% each). In virtual absence of systemic side effects, local reactions were generally tolerable. A decrease in the hazard rate for local reactions with increasing duration of treatment indicates that there is no significant risk for a sensitization. Conclusions: Miltefosine solution is an effective and tolerable new treatment modality, especially in patients with cutaneous lesions with limited depth of infiltration. Prognostic factors identified by this analysis may serve as an aid in treatment planning for individual patients, but also as a data base for future, confirmatory trials.

Das neue Antiöstrogen Droloxifen (DROL) unterscheidet sich pharmakokinetisch von Tamoxifen (TAM) durch eine rasche Aufnahme, geringe Akkumulation und eine kurze Eliminationshalbwertszeit. Diese charakteristischen Eigenschaften von DROL sind Voraussetzung für ein intermittierendes Therapieschema, welches unter Umständen die Resistenzentstehung verhindern und mögliche Nebenwirkungen der adjuvanten Langzeittherapie weiter reduzieren könnte. Wir verglichen die Wirkungen von Kurzzeit- und intermittierender Anwendung von DROL und TAM auf menschliche Tumorzellinien durch Messung des gesamten DNS-Gehaltes von Kul-turschalen mit der Burton-Reagens-Methode. An beiden Östrogenrezeptor(ER)-positiven Brustkrebszellinien, ZR-75-1 und MCF-7 Ml, wirkte DROL viel schneller als TAM, was auf eine schnellere Aufnahmekinetik hindeutet. DROL brauchte nur 15-30 min Inkubationszeit, um seine voile antiproliferative Wirkung zu entfalten, während bei TAM, das außerdem deutlich weniger wirksam war, wenigstens 2 h not-wendig waren. Eine einzige 2stündige Inkubationszeit von MCF-7-Ml-Zellen mit DROL hemmte das Zellwachstum vollständig bis zu 11 Tagen. Die Simulation eines intermittierenden Therapieschemas in vitro durch 2stündige Behandlungsintervalle jeden 3. Tag resultierte in völliger Wachstumshemmung von MCF-7-Ml-Zellen über mehr als 3 Wochen. Im Vergleich zu DROL zeigten sowohl TAM als auch Toremifen eine wenigstens lOmal schwächere antiproliferative Wirkung auf ER-positive Zellen. Diese Differenz wurde noch größer, wenn kurze Substanzinkubationszeiten angewendet wurden. Aufgrund der schnellen Aufnahmekinetik und der anhalten-den Wachstumsinhibition nach Kurzzeitapplikation von therapeutisch relevanten Konzentrationen ist DROL das erste Antiöstrogen, das sich für ein intermittierendes Schema der Hormontherapie bei Brustkreb
