In a prospective randomized trial, the pending question was addressed whether Cytosine arabinoside (Ara-C) should be applied at high or intermediate dose to patients with relapsed or refractory acute myeloid leukemia. Based upon the previously established regimen of the sequential application of Ara-C and Mitoxantrone (S-HAM) patients below 60 years of age were randomized to receive Ara-C at either 3.0 g/m2 vs 1.0 g/m2 per dose while older patients were randomly assigned to either 1.0 g/m2 or 0.5 g/m2 Ara-C. At the present early stage 51 patients have entered the study and 37 are currently evaluable for response and toxicity. Complete remissions were achieved in 14 of 28 patients below 60 years of age and in 3 of 8 older cases. Predominant side effects consisted of nausea and vomiting, diarrhea and stomatitis. Further recruitment of patients and longer follow-up is required for the assessment of the various treatment arms.

Kürzlich wurde ein neuer spezifischer Test zur Krebserkennung vorgestellt. Dabei wiesen die Autoren durch NMR-Spektrometrie eine signifikante Abnahme der Linienbreite der Lipoproteine im Plasma von Patienten mit malignen Tumoren nach. Die vorgelegte kooperative Studie wurde als kritische Prüfung des sogenannten Fossel-Tests angelegt. Wir untersuchten 100 Proben, 27 von gesunden Probanden, 13 von schwangeren Frauen, 13 von Patienten mit benignen Erkrankungen und 47 von Patienten mit histologisch gesicherter maligner Erkrankung ohne Primärtherapie. Die Probeentnahme erfolgte morgens an nüchternen Patienten. Anschließend wurde das EDTA-Plasma bis zur spektrometrischen Messung bei minus 18°C gelagert. Die NMR-Spektrometrie erfolgte an einem AM-Spektrometer. Zur Wasserunterdrückung wurde ein Entkoppelungsverfahren und als Locksubstanz Deuteriumoxid verwendet. Die mittlere Linienbreite der Lipoproteine für Gesunde betrug 41,07 ± 6,08 Hz und 34,40 ± 10,80 für Patienten mit nicht-malignen Erkrankungen. Patienten mit malignen Erkrankungen zeigten eine mittlere Linienbreite von 35,40 ± 8,0 Hz. Auch im Plasma der Schwangeren fand sich eine signifikante Abnahme auf 32,60 ± 5,7 Hz. Die Studie zeigte z.T. signifikante Unterschiede in der mittleren Linienbreite der Methyl- bzw. Methylengruppen der Lipoproteine im Plasma von Gesunden und Patienten mit Mal·ignomen. Im Gegensatz zur Origínalmitteilung fanden wir eine breite Überlappung der Bereiche. 69 patients (median age 53 years, 19–79 years old) with untreated acute non-lymphoblastic leukemia (ANLL) were randomized to receive either a regimen of amsacrine, cytarabine, thioguanine (AAT) or daunorubicine. cytarabine, thioguanine (DAT). AAT consisted of amsacrine 200 mg/m2/day × 5, thioguanine 100 mg/m2/12 h p.o. × 10; DAT was daunorubicine 50 mg/m2/day × 3, cytarabine 200 mg/m2/day × 5, thioguanine 100 mg/m2/12 h p.o. × 10. After one or two induction courses the patients subsequently received 2 consolidation courses. 17 patients were not assessable for response to therapy due to exitus during induction treatment. Complete remission could be obtained in 14/24 (58%) of DAT patients respectively. Patients < óO years of age achieved CR in 63% (AAT) vs 65% (DAT), whereas patients ≥60 years obtained a CR in 50% (AAT) vs 13% (DAT). Zur Identifízierung von Neuroblastomzellen aus Knochenmark von 12 Kindern in verschiedenen Krankheitsstadien wurde das immuno-alkalische Phosphatase-Verfahren (APAAP) eingesetzt. Unter Verwen-dung eines Panels monoklonaler Antikörper, die sich selektiv an Zellen neuroektodermalen Ursprungs anbinden, wurden 72 immunologische Analysen durchgeführt. Hierbei wurden 57 × Tumorzellen identifiziert, wohingegen histologische und zytologische Analysen in 45 beziehungsweise 37 Fallen pathologische Zellen zeigten. Die Reaktivität kleinster Resttumorzellen – in der Hauptsache mit den drei monoklonalen Antikörpern UJ13A, UJ167.11, A2B5 – weist darauf hin. daß diese Zellen zu einem resistenten Neuroblastomzellklon gehören, der trotz intensiver Therapie im Knochenmark verbleibt. In a pilot study, five patients with myelodysplastic syndromes with an excess of blast cells were treated with a combination of recombinant human granulocyte-macrophage colony-stimulating factor (rhGM-CSF) and low-dose cytosine-arabinoside (ara-C) in an attempt to selectively kill the leukemic blast cells and thereby to restore normal hemopoiesis. The treatment schedule consisted of three 14-day-cycles of 250 μg/m2 rhGM-CSF and 20 mg/m2 ara-C given daily s.c. with four-week treatment-free intervals. In all four evaluable patients the percentage of bone marrow blast cells decreased significantly with an increase in the mature myeloid cells but without bone marrow aplasia. Toxic side effects attributable to the drugs were minor with fever, mild bone pain, erythema and itching at the site of subcutaneous injection of rhGM-CSF. GM-CSF was used to overcome fatal myelosuppression after cytotoxic chemotherapy. Two different application modalities were compared: a continuous 24 h infusion was more effective compared to a 30 min short term infusion. Using the former modality at a dose of 10 μg/kg/d for five days very impressive responses were observed. No major side effects did occur. The first 13 patients treated in this way included 8 AML cases. Only one of these latter patients had a leukemia relapse. However, in this patient the immediate GM-CSF response was clearly separated from the relapse occurring later in the observation period. Thus, the preliminary results of the present paper suggest that GM-CSF besides of being very efficient in accelerating granulocyte recovery does not stimulate the growth of AML blasts in vivo in patients with only minimal residual disease
