It is difficult to imagine a more perennially vexing topic to philosophers, scientists, and physicians than the mind-body problem. Recent literature bears out its continued vital interest for psychiatrists. This article briefly recapitulates the major perspectives on the problem, examines the relationship of meaning and mind to psychosocial and biological explanatory programs and to materiality, and promotes a monistic dual aspect interactionist approach to mind and body in health and illness. From this thesis conclusions are drawn in regard to the ultimate possibility of a psychiatric unitary field theory, the question of the autonomy of the psychological and biological explanatory programs, and the identity of the psychiatrist.

This study examined the utility of the DSM-III diagnostic criteria for posttraumatic stress disorder (PTSD) in a high-risk group of 50 firefighters who had had an intense exposure to a natural disaster 8 months before being interviewed. Follow-up over the next 3 years allowed examination of the ability of these diagnostic criteria to predict a pattern of chronic posttraumatic morbidity. They predicted a pattern of chronic disorder, demonstrated by the finding that eight of the 15 subjects who had definite or borderline PTSD at 8 months remained symptomatic 3 years later. A disturbance of attention and concentration appeared to be the best predictor of chronic PTSD. The longitudinal course of posttraumatic morbidity in these 50 firefighters was compared with a matched group of 96 uninterviewed subjects 11 and 29 months after the disaster. Although the interview provoked an emotional catharsis in a number of firefighters, the long-term morbidity in the two groups was comparable. Fourteen subjects who did not have PTSD continued to experience intense imagery 8 months after the disaster. This observation raises questions about whether such thoughts and feelings have adequate specificity as diagnostic criteria for PTSD in a group that has recently been exposed to a traumatic event.

This study examined the longitudinal course, over a 25-month period, of posttraumatic morbidity in a group of 469 firefighters exposed to a bushfire disaster. The patterns of posttraumatic morbidity were defined by the General Health Questionnaire. Contrary to expectation, an acute pattern of morbidity was less common than the delayed-onset or chronic forms. Predisaster variables were found to be as important in the onset and course of the disorder as were the firefighters' losses or extent of exposure to the disaster. These data suggest that exposure to an extreme trauma is necessary but not sufficient to explain the onset and pattern of posttraumatic morbidity.

Fifteen patients with a diagnosis of borderline personality disorder (BPD) who had committed suicide while under inpatient care or within a month after discharge were compared with a group of 13 inpatients with a diagnosis of BPD who did not kill themselves. Suicides occurred in all ages from 20 to 49 years. Age, sex, and age when first in contact with psychiatry did not differ between groups. DIB profiles differed only with respect to slightly higher scores on the affect section among patients who committed suicide. Axis I affective disorders were equally frequent. The patients who committed suicide had been more often hospitalized and they had made more suicide attempts in their lifetime. Male patients who killed themselves showed a more extensive suicidal behavior at admission than did their matched control subjects. Earlier suicide attempts during inpatient treatment were only identified among the patients who committed suicide. An imminent mandatory discharge preceded the inpatient suicides in five of 11 cases.

This report investigates the way in which the presence of a personality disorder (PD) in probands affects the frequency of psychiatric disorders in relatives. Four proband groups– panic, panic plus a PD, major depression, and depression plus a PD–were given the Family History-Research Diagnostic Criteria and the Family History for DSM-III Anxiety and Personality Disorders. Information was gathered on 730 first-degree relatives. Panic patients with a PD had higher incidences of panic and panic plus a PD in relatives. There was increased depression in relatives of probands witheitherdepression or a PD. Alcoholism appeared highest in relatives of probands who had combined depression and a PD.

This single case study reports on a 74-year-old woman with 48-hour cycles of mood disturbance for 2 years. Every other day she awakened feeling sad with low energy, decreased appetite, fatigue, diminished enjoyment of normal activities, increased irritability, occasional self-deprecatory thoughts, and difficulty concentrating. On alternative days she was active, outgoing, energetic, and cheerful. Her past history was marked by mild postpartum depressions, not requiring treatment, with each of her six pregnancies, and a more severe depression at age 57, which seemed to respond to Premarin. During the recent 2-year period of mood swings, the patient was given trials of several heterocyclic antidepressant medications, but side effects precluded the use of therapeutic doses or durations of treatment. Reluctantly, the patient agreed to a trial of lithium carbonate. After the second week of lithium treatment, at a level of 0.4 mEq/ml, she reported marked improvement, feeling fine every day without mood fluctuations. After almost 1 year at this blood level, she remains asymptomatic. Thus, the patient's cyclic 48-hour unipolar depression responded dramatically and completely to low doses of lithium carbonate.
