HypothesisThis study hypothesizes that a suburethral sling, placed with minimal tension and no intent to alter urethral position, will restore the urethra to a more retropubic position in the same fashion as a urethropexy.Materials and MethodsTwenty-nine women were prospectively followed-up with during their evaluation and treatment for stress urinary incontinence. All of the patients underwent a pubovaginal sling with autologous abdominis rectus fascia. Using a cotton swab and goniometer, all patients had their resting urethrovesical angle measured before and at 1 and 3 months after surgery. The postoperative change in resting urethral angle in the patients was quantified and compared in three groups, which were divided based on the preoperative incontinence diagnosis.ResultsThe change in the resting urethrovesical angle caused by sling surgery was statistically significant. Preoperative incontinence diagnosis did not impact on the degree of the postoperative static urethral angle change.ConclusionA loosely tied sling alters the resting urethrovesical angle and increases suburethral support like a backboard while it also restores the urethra to a more retropubic position. This is similar to urethropexy.

ObjectiveTo examine the surgical and functional success of complete vaginal-cuff excision for transvaginal repair of vesicovaginal fistulae.Material and MethodsThirty-four women with posthysterectomy vesicovaginal fistulae presented for repair. Surgical technique included vaginal-cuff scar excision and fistula tract removal with a multilayer closure. The last 14 patients completed questionnaires and underwent pelvic prolapse staging (POP-Q) pre- and postoperatively.ResultsThirty-nine percent of the women had previous attempts at closure of the fistula. Patients presented at an average of 24.6 weeks (range, 4–52 weeks) after the initial injury or a previous attempt. All patients were permanently dry. Fistula repair did not compromise ability to have sexual activity nor affect comfort during intercourse, and total vaginal length did not change. Nine percent of the women (3/34) had minor urinary urgency symptoms.ConclusionTransvaginal vaginal-cuff excision is an effective technique to repair posthysterectomy-induced fistulae. It preserves vaginal function and is not associated with urinary complications.

ObjectiveTo compare postoperative anatomic cure rates in women treated with bilateral sacrospinous ligament fixation (SSLF) or unilateral SSLF for pelvic prolapse. Secondary outcome measures of morbidity were compared.MethodsOne hundred and three women with pelvic prolapse treated surgically between July 1, 1990, and September 31, 1999, were divided into two groups: patients with bilateral SSLF (n = 62) and patients with unilateral SSLF (n = 41). Demographic, clinical, and outcome data were compared.ResultsThere were similar anatomic cure rates regardless of SSLF type (37/41 women [90.2%], unilateral, compared with 53/62 [85.5%], bilateral;P= 0.56). Specific anatomic defects occurred with similar frequencies between the groups. Women in the unilateral SSLF group had more blood loss and longer operative times than did those in the bilateral group. There were similar rates of transfusion, readmission, cystitis, postoperative incontinence, urinary retention, febrile morbidity, and length of stay between the two groups.ConclusionUnilateral and bilateral SSLF techniques produce similar rates of anatomic cure.

The Swiss surgeon Emil Theodor Kocher (1841–1917) received the 1909 Nobel Prize in physiology or medicine for his work on the thyroid gland. This innovative work eventually resulted in effective treatment of thyroid disorders not only by the use of surgery but also by the use of such drugs as thyroid extract and thyroxin.
