A survey was carried out among a sample of 973 nurses in hospitals in the Valencian Community (Spain). The aim was to study the relation between work organization, sociodemographic and professional factors and the mental health of nursing staff. The information was collected by means of a questionnaire. Three indicators were used to study mental health: fatigue, sleep disturbance and psychological or psychosomatic indicators of mental suffering. Work organization affected the mental health of workers with varying impact according to the mental health indicator under consideration. Dissatisfaction due to work schedule, psychological and mental load increased the risk of showing signs of fatigue, with a prevalence odds ratio (POR) of 2.15, 2.00, 1.86, respectively, along with the risk of psychological symptoms with a POR of 2.36, 1.80, 2.10, respectively. Permanent night shifts or rotating night shifts increased the risk of sleep disturbance with a POR of 2.00 and 1.34 respectively.

This descriptive study measures the attitude of ward based psychiatric nurses (n=59) towards nursing. Individual's attitude scores are correlated with the variables clinical grade, rank, age, time spent working on the same ward and attendance at the Open University courseSystematic Approach t o Nursing Care. Although some trends are identified none of the variables positively correlated with attitude toward the nursing process. Additionally, factors in the formation and maintenance of attitudes toward the nursing process of a small group of senior ward nurses (n=3) are explored using a structured interview schedule. Qualitative analysis of the interviews suggests a lack of clinical staff involvement in the introduction of nursing process and lack of activities aimed at maintaining positive attitudes toward the nursing process. The results are discussed in terms of Ajzen's (1988) attitude theory and Handy's (1985) organizational change theory. Recommendations are made concerning the introduction of future changes to the organization of the delivery of client care.

A methodology for Patient Costing is presented, which is based upon a division of nursing care into Direct, Indirect and a residual category, and which is consistent with costing methods recommended by the Audit Commission. The system was used to produce patient costs upon 14 wards in Wales. The results show wide differences in costs, both between and within wards. The House of Commons Select Committee 2nd Report on Maternity Services (1992) provides recommendations to the future structuring of Maternity Services. The explicit messages provided by this report indicate that services should be woman centred, placing the Midwife as the Central Care Provider. Politically this places midwives in a strong position to lead future maternity services. The current culture that maternity services are operating needs to be examined by Managers before the recommendations can become operational policy. Attention also needs to focus on those elements of practice and organization that currently meet the report's recommendations. This is essential to ensure that any re‐organization required builds upon the strengths within maternity services whilst addressing the weaknesses. At Leicester Royal Infirnlary the development of a Midwifery Managed Service has evolved over a period of 5 years. The developments at this Unit have provided foundations to take the Winterton Committee's recommendations forward in a positive structured process that will guarantee consumer controls whilst establishing midwives as leaders in maternity service provisio

This paper focuses on nursing research and its management in Britain, examining the roles of nurse managers, researchers, clinicians and educationalists in this context. The background to commissioning health research within the Department of Health (formerly Department of Health and Social Security) is outlined together with the main milestones in NHS based research from the 1970s until the present time. Emphasis is returned to nursing research through a systematic literature search of the most popular British nursing journals, over 18 months (Nursing Times, Nursing Standard, Senior Nurse and lournal of Advanced Nursing), exploring various aspects of research management. Comparative material is drawn from the American nursing journals, the lournal of Nursing Administration and Nursing Management. The article is concluded with reference to challenges presented in Research for Health (The Strategy for Research and Development in the NHS 1991) and in the forthcoming Strategy for Nursing, Midwifery and Health Visiting Research. Emphasis is placed on strengthened collaboration between nurse managers, researchers, clinicians and educationists, in order to fulfil the aims established for research in all sectors of the NHS and, particularly, within nursing research.
