摘要:
Team building should not be a ‘bolt‐on’ extra, it should be a well planned, integrated part of developing teams and assisting their leaders. When asked to facilitate team building by a group of NHS managers we developed a framework which enabled individual members of staff to become more effective in the way they communicated with each other, their teams and in turn within the organization. Facing the challenge posed by complex organizational changes, staff were able to use 3 training days to increase and develop their awareness of the principles of teamwork, better team management, and how a process of leadership and team building could help yield better patient

This paper presents an overview of a Department of Health and Community Services funded study into the application of the ‘Omaha’ data management system (Martin&Scheet 1992) to the work of district nurses in urban, rural and remote areas of south west Victoria. The data indicate a direct correlation between the complexity of the clients' demands for care, their dependency on the nursing service and the time taken to complete an episode of care. A large scale study of neonatal nursing was carried out with data collected from 56 sites (4 hospitals from each of the health regions of England). In the area of skill mix the key findings were:.the largest segment of the neonatal nursing workforce is made up of staff nurses and staff midwives, and of these there are greater numbers of staff nurses; the E grade at 31% is the most frequently found, followed by the G (19%) and F grades (16%); vacancies were disproportionately high at staff nurse/staff midwife level; nursery nurses were employed for direct nursing care in 80% of units; nursing auxilliaries were employed in 70% of units, though in only half were they involved in the direct care of babies. Staffing policies were diverse and not clearly related to the type of unit or the numbers of designated intensive and special care cots. There was a tendency to employ more junior staff at night, the use of support staff varied widely, and in many units was at very low levels or absent. In addition to differing service demands that may affect the skill mix required, there is a clear need for the profession to address the questions that arise from changing roles and areas of responsibility in the spheres of management, teaching and clinical practice.

This paper reports the principal findings of an initiative in Wakefield, UK, to implement a case management approach to the resettlement of individuals from a long‐stay psychiatric hospital. Nurses and their managers, while welcoming the needs‐led, user‐centred orientation of such an approach, found some difficulty with it in practice. The principal lesson to be drawn from the experience is that there is still much to learn about case management within the context of British health and social care. This paper examines the factors leading to the introduction of a competitive market in the British National Health Service (NHS). It can be seen that this most radical overhaul in the 45 years of the service was precipitated by economic but also ideological factors. Competition was seen as a means to producing the efficiency that would lead to better health care. The competitive market was introduced without public debate or reference to similar experiences in health care provision in the USA. Littl consideration was given to the strengths and weaknesses of such a system. The purchaser‐provider model chosen has been expensive to introduce. It has failed to stimulate competition amongst providers and the formal economic evaluation of its performance has so far been minimal. At first sight the separation of purchaser‐provider functions has done little to disturb the traditional bureaucratic features of the NHS. The scope exists, however, to open up competition to a much greater extent. Depending on political feasibility, competition would be the necessary mechanism to prepare the NHS for sale into private owners

摘要:
The recent National Health Service (NHS) reforms are having a fundamental impact upon the culture, structure and financing of the NHS, and the way in which patients receive care. The challenges this presents for the future of nursing practice, management and leadership are profound. This paper provides an account of how one NHS region, South West Thames, has sought to identify and develop the competencies required of future NHS trust nursing directors, and other top managers within the region, by participation in a sophisticated diagnostic process known as the Development Centre. The paper firstly explores the background to the Region's Career Development and Planning System (CDPS), then outlines the process of the identification of competencies, before exploring in detail the six stages of the Development Centre diagnostic process.
