Low molecular weight (40,000) dextran is being extensively used in open heart surgery to minimize the amount of donor blood required and to inhibit red cell aggregation. At equal concentrations, its effects on blood coagulation were less than those of standard dextran plasma substitutes; but in the very high concentrations used in open heart surgery its effects were greater. Addition of low molecular weight dextran to normal plasma caused a gross precipitate and striking inhibition of coagulation. Factor VIII (antihemophilic factor) and fibrinogen were partially removed with the precipitate. Patients subjected to open heart surgery had lower levels of these two clotting factors in their plasma when dextran was used, but it is not certain whether this loss occurred in vivo, or during separation of the plasma from red cells (and precipitate) prior to testing. No proof was obtained in this series of cases that low molecular weight dextran increased the tendency to bleed, but the results indicate that this would be a likely possibility with the use of larger amounts.The low molecular weight dextran had a slight tendency to induce red cell aggregation by itself. It potentiated the aggregative effect of Polybrene and the agglutinating effect of a saline Rh antibody. On the other hand, it inhibited agglutination of red cells induced by an Rh antibody in a high protein medium. It thus appears likely that high concentrations of low molecular weight dextran in the blood of a patient could lead to erroneous conclusions in compatibility tests.

An anti‐Rh gamma2‐globulin antibody preparation has been developed which can be administered intramuscularly and appears to be both safe and effective in the prevention of experimental Rh sensitization. Nine unsensitized Rh‐negative male volunteers were challenged once a month for five successive months with intravenous injections of 2 ml. of Rh‐positive blood. Four of these nine volunteers were passively protected each month with intramuscular injections of 5 ml. of this antibody preparation, administered 24 hours prior to the antigenic challenge. Three months after the last injection the passively acquired Rh antibodies were no longer demonstrable (by either the saline or indirect antiglobulin technics) in any of the four protected subjects and there was no sign of active antibody production six months after the last injection, whereas four of the five controls were all strongly sen

A new device for simultaneous withdrawal and injection of blood during exchange transfusion in the newborn is presented. The advantages over previous methods consist in that it is more physiological preventing overloading or decrease of the circulating blood, and it is practically as effective in removing baby's red cells as the intermittent procedure.

Serum samples from 1,258 normal adult blood donors were assayed for complement activity. The average complement titer for the entire sample was 38.14. There was no difference in complement activity with respect to sex, blood group or Rh***(D) type. The level of circulating complement appears to be higher in older humans and in heavier individuals.In this kind of experimentation, the need for a standard complement for assay of different batches of sheep erythrocytes is emphasized.

The accuracy of the copper sulfate method of determining hemoglobin was investigated under usual blood bank conditions. In our hands this method was found to be grossly inaccurate. Using the cyanmethemoglobin technic, it was demonstrated that 83.9 per cent of females and 80.4 per cent of males rejected as having a low hemoglobin by the copper sulfate method, actually had hemoglobin values above minimum requirements for blood donors. Detailed instruction as to technics and pitfalls of the copper sulfate method was ineffectual in correcting errors as encountered by a large nursing staff carrying out the procedure. Donors found to have acceptable hemoglobin values by the copper sulfate method appeared to be safely accepted as donors; cyanmethemoglobin duplicate checks did not reveal any discrepancies in this group.
