The properties of four albumin‐agglutinating sera demonstrating a spectrum of reactivities are reported. All cause pan‐ and auto‐agglutination in albumin. Direct and indirect reacting caprylate‐dependent agglutinins are described, as are two sera with undefined specificity not related to the presence of caprylate stabilizer in the albumin. Immunoelectrophoretic evidence was obtained to support the concept of albumin‐immunoglobulin complex formation in the genesis of albumin agglutination. Most albumin agglutinins react only with albumin stabilized by short‐chain fatty acids; however, other specificities may be represented. Platelet function tests (bleeding time, platelet adhesiveness, platelet factor 3 release, and platelet aggregation to ADP) were done on a group of severe factor VIII deficient hemophiliacs before, during, or after transfusion therapy with cryoprecipitate or factor VIII concentrate. Mild decreased platelet function was seen in nonbleeding hemophiliacs after a single transfusion of cryoprecipitate. More severe abnormalities of platelet function were observed in a group of patients receiving factor VIII therapy for severe hemorrhages or in the postoperative period. Several of these intensively transfused patients bled when their circulating factor VIII level was normal or near normal. In one patient, who was studied repeatedly, prolongation of the thrombin time and partial thromboplastin time occurred in association with high levels of fibrinogen. Fibrin split products (fsp) were increased in most of the patients during bleeding episodes. Mixtures of cryoprecipitate or factor VIII‐concentrate with normal plasma contained increased protamine precipitable material and fibrin split products. 51Cr labelled red blood cells were transfused into nine patients during surgery, and the rate of chromium elutionin vivowas determined. Similar studies were performed on seven nonsurgical patients. Three of the surgical and two of the nonsurgical patients received liquid‐stored red blood cells, while six of the surgical and five of the nonsurgical patients received previously frozen agglomerated red blood cells. The rapid component of chromium elution was observed in both surgical and nonsurgical patients. Approximately 6.7 per cent of the chromium eluted rapidly from liquid‐preserved red blood cells, and about 9.9 per cent from previously frozen red blood cells. No further elution was detected in the red blood cells transfused during surgery, whereas further elution (the slow component of51Cr elution) was observed in nonsurgical patients. A 68‐year‐old male with nephrotic syndrome due to diabetic glomerulosclerosis was found to have high serum alkaline phosphatase after albumin infusions. By examining several albumin preparations, certain products were found to contain as high as 200 Sigma units of alkaline phosphatase per ml. By disc electrophoresis and heat treatment (56 C for 30 min), all of the alkaline phosphatases in the albumin preparations were found to be heat stable and of placental origin. Since a high alkaline phosphatase activity in serum may mislead diagnosis, it is quite important to know whether the high level of the enzyme in patient's specimens is of exogenous or endogenous origin. It is recommended that sera obtained from patients who have been treated with albumin should be heat tested before determination of alkaline phosphatase. The agglutinability of red blood cells from patients with various hematologic disorders was compared with that of erythrocytes from normal persons by quantitative hemagglutination in the ABH blood group system. Red blood cells from six patients with hypoplastic anemia or metastatic marrow disease as well as normal reticulocytes (five experiments) were less agglutinable than normal mature cells. Erythrocytes from one patient with lymphoma and an acquired hemolytic anemia showed enhanced agglutinability. Our data suggest that changes in erythrocyte reactivity may be more common than suspected, although usually undetected by less sensitive methods.

A positive donor recipient identification system for a transfusion service has been developed. Donor and recipient identification are established by a human and machine readable number. The recipient ID system utilizes the standard hospital ID bracelet and a series of numerical thermal encoding transfer devices for vacutainer tubes and donor unit ID bag tags. The donor identity is maintained by prenumbered vacutainer pilot tubes and a separate donor unit bag tag.

A pair of monozygotic erythroblastotic twins, one with hydrops, the other without, were treated by intrauterine transfusion. The twins had a monochorionic, diamniotic placenta with gross surface vascular anastomoses; at birth their circulating blood was of a single composition. The composition established that the presence of ascites per se does not inhibit absorption of blood from the peritoneal cavity.

A follow‐up statistical analysis of the 1971 College of American Pathologists Comprehensive Blood Bank Survey was made to determine if the variability of blood bank reagents noted in the 1970 Survey had significantly changed between 1970 and 1971.
