摘要:
The purpose of this descriptive study was to look at foodservice employees' perceptions of the importance of specific meal service factors in providing quality meal service to residents of nursing facilities. Data were collected from 112 respondents. The factors rated as essential by the total sample were in the category of managing personnel. All other categories were rated as important by the total sample. Significant differences (p 5 .05) were found between consulting dietitians and dietary managers in the category, using work simplification and between foodservice supervisors and dietary managers in the category, creating a pleasant dining room atmosphere. No significant differences were found between consulting dietitians and foodservice supervisors.

A large percentage of long-term care residents receive enteral feedings. A study was conducted to determine the extent to which residents were receiving amounts prescribed by physicians and enteral feeding contributions toward residents' nutritional needs. During a three week period, 48 hours each week, a case series was generated from 64 residents who were enterally fed in eleven long-term care facilities. Results showed that only residents who received intermittent feedings received 100% of their prescribed diet order each day. The number of hours per day an enteral feeding was stopped ranged from 0 hours to 8.6 hours. About two thirds (68%) of the residents received 90% or more of their prescribed order. Seven percent of the residents received 77% or less of the prescribed amount. Eighty-seven percent of the residents' actual intake was 100% or more of their basal energy expenditure (BEE), while 7% received 80% or less than their calculated BEE. Findings suggest that dietitians and nurses in long-term care should more closely monitor enteral intakes to assure both compliance with physician orders and fulfillment of residents' nutritional needs.

An eight-week nutrition education program on sodium status and health was investigated in 41 elderly subjects; changes in knowledge and status of selected minerals were assessed. An experimental design was used with four groups: hypertensive treatment (HT), hypertensive control (HC), normotensive treatment (NT) and normotensive control (NC). The program was effective in increasing knowledge in the HT and NT with a significant gain in pre to posttest scores, (p l 0.01 and p 5 0.05, respectively). A post-posttest, given six weeks after intervention, showed that HT retained more knowledge than NT, (15% and 6%, respectively). Dietary calcium increased in HT and NT and potassium decreased in HT (p<0.05) but not in NT. Urinary sodium and dietary sodium decreased in the HT, levels of sodium and potassium increased slightly in the NT group. The education program was more effective in the HT group than the NT possibly indicating that elderly with a medical need for dietary modification are more positively affected by an education program than elderly without a medical need.

This study explores those variables that shape (a) awareness of nutrition programs and (b) actual use of nutrition programs among a sample of rural elderly in New York State. Evidence indicates that program awareness is quite high and that, compared with evidence from New York State as a whole, program use IS rather high. Those factors shaping both program awareness and use are identified. Then, policy implications are discussed.
