Purpose:To report four patients with unusual multifocal patterns of small fiber sensory neuropathyMethods:Four patients who developed multifocal sensory symptoms after infectious illnesses underwent neurologic examinations, nerve conduction studies, and serologic testing. All had one to three types of electrodiagnostic assessments of small nerve fibers by evaluation of thermal and vibratory thresholds. quantitative sudomotor axon reflex testing, or autonomic cardiovascular testing.ResultsNerve conduction and serologic studies were normal. Quantitative sudomotor axon reflex testing, autonomic cardiovascular testing, or both revealed abnormalities of small fiber function in all patients, and thermal thresholds were abnormal in one. Three had progression and fluctuations of symptoms for months.Conclusions:Small fiber sensory neuropathies occasionally occur in a multifocal rather than the typical distal distribution and may comprise part of the spectrum of postinfectious sensory neuropathies. Objective testing of small fiber function is useful in distinguishing small fiber sensory neuropathy from other causes of paresthesias in this setting.

Wartenberg's migrant sensory neuritis (WMSN) is an unusual disorder characterized by the occurrence of multiple mononeuropathies involving cutaneous sensory nerves. The duration of the disease is often one of several years, and the overall course is usually a benign one. We present three cases of WMSN and describe I the clinical and electrodiagnostic findings.

A 73-year-old man presented with cervical cord compression resulting from a metastatic spindle cell thymoma 10 years after undergoing thymectomy for thymoma-associated myasthenia gravis. After being virtually asymptomatic following thymectomy, his myasthenic symptoms returned 3 months after removal of the spinal metastasis, culminating in myasthenic crisis that required immunosuppression and plasma exchange This case shows that isolated, extrathoracic metastases without any mediastinal tumor may occur many years after apparent surgical cure of primary thymoma, and that myasthenia gravis may recur after surgical resection of a thymic metastasis.

We report an unusual association of peroneal mononeuropathy at the fibular head and the use of antithrombotic stockings. A patient developed clinical evidence of a peroneal nerve deficit temporally related to the use of a tight-fitting antithrombotic stocking. Nerve conduction studies and needle electromyography were performed. A peroneal mononeuropathy at the fibular head was confirmed and demonstrated mixed axonal and demyelinatmg features. Antithrombotic stockings are an unusual cause of peroneal mononeuropathy, Early recognition of this complication may improve outcome.

Inflammatory neuropathies represent a wide spectrum of disorders with important differences in immunologic mechanisms Effective treatment and accurate prognosis is based on accurate diagnosis from clinical laboratory and electredugnostic data. Selection of treatment options should be guided by evidence-based data (randomized controlled trials)- In this article we review the treatment of the various forms of Guillain-Barre syndrome and chronic inflammatory demyelinating polyradiculoneuropathy and data supporting each modality.

Paraneoplastic disorders ace uncommon but clinically important complications of a large number of neoplasms. Most paraneoplastic syndromes are thought to have an autoimmune etiology, although the weight of evidence supporting autoimmunity vanes among syndromes. Prompt diagnosis of a paraneoplastic disorder can increase the likelihood of a favorable neurologic and oncologic outcome. This article reviews the; clinical features, autoimmune aspects, diagnostic approach, and treatment options for patients with paraneoplastic disorders affecting the spinal cord, motor neurons, neuromuscular junction, and muscle. Part 2 of this review, to be published in the next issue, will discuss the paraneoplastic neuropathies.
