AbstractThe expected growth in numbers of very elderly people, some of whom will be frail, in the United Kingdom and elsewhere may lead to a reappraisal of policies. It is argued that housing has a crucial role to play. Research evidence is presented that most elderly people wish to remain in homes of their own and ways in which this can be achieved are discussed. For those who wish, or have, to move, a number of options are outlined. These include ordinary small housing, living with a family, granny annexes, sheltered housing, extra care housing, continuing care communities and residential care. If elderly people are to live in an appropriate setting all professional groups need to cooperate.

AbstractOne hundred and sixty‐three patients treated with electroconvulsive therapy (ECT) in a district psychogeriatric unit during 1981 were followed up after 3 years. After the original course (mean 11·2 treatments) 51% were rated as fully recovered and 23% as much improved. Two‐thirds experienced one or more relapse within 3 years but half of these regained full remission. Of the 113 patients alive at 3 years 59% were free of depression and 29% showed mild symptoms only. Those making full initial recoveries had fared best subsequently. AbstractAll patients receiving electroconvulsive therapy (ECT) under the care of the Old Age Psychiatry service at the University Hospital of South Manchester between 1976 and 1982 were studied. Data on 193 courses were collected. Eighty per cent of patients were well or considerably improved at discharge. When relapses occurred they usually responded well to antidepressant treatment and did not indicate a poor outcome in the long term. The results confirm that ECT is a safe and effective treatment for elderly patients with severe depressive illnesses, despite the fact that many of them suffer from concurrent physical problems and that some have evidence of intellectual impairment.

AbstractThis article describes a survey undertaken in 1976 of the prevalence of psychiatric illness in a total population of people aged 65 or over in the district of Clackmannan, Scotland. Using the Survey Psychiatric Assessment Schedule (Bondet al., 1980) prevalence rates of 7% for organic disorders and 22·5% for affective disorders and psychoneuroses were estimated for the total population of elderly people living at home and in institutions. This article reports logistic discriminant analyses which identify those variables which together predict the presence of psychiatric illness. Together age, functional capacity, civil status, the absence of an affective disorder or psychoneuroses and a visual impairment predict correctly whether a person has an organic disorder for 85% of the population. AbstractThe performance of the Dexamethasone Suppression Test (DST) was investigated in 60 demented patients including 14 patients admitted to a psychogeriatric assessment ward, 29 electively admitted patients and 17 long‐stay patients in comparison with 37 normal control subjects. It was shown that acutely admitted patients had the highest rate of DST non‐suppression (86%), whilst electively admitted patients had the lowest rate (55%) in comparison with normal controls (11%). In the electively admitted group abnormal DST results and Cortisol Suppression Index and DST non‐suppressors showed depressive signs more frequently than suppressors.
