AbstractThe Guy's/Age Concern Home Support Project is a prospective controlled intervention study of the effect of client‐centred home support for elderly people with dementia. Control subjects who were in institutions at six months follow‐up were significantly more depressed and physically disabled at referral than those who remained at home. Among those receiving additional home support, however, there was no difference between the depression and disability scores at referral of those who were institutionalized and those who remained at home at follow‐up. These results suggest that the high prevalence of depression and physical disability in the institutionalized elderly is due to preferential selection into institutions, and that this process is prevented by providing this form of domiciliary su

AbstractA drug utilization study was carried out in 11 geriatric institutions for independent or dependent people in Padua. Clinical records and treatment charts of the 1533 residents were reviewed and details on sociodemographic characteristics of the population and prescription of psychotropic drugs were recorded. Mean age of the residents was 79 years and mean length of stay five years; women were 78% of residents. Psychotropic drug prescriptions were recorded in 26% of the residents; frequency of prescriptions of benzodiazepines, neuroleptics and antidepressants was respectively 21%, 8% and 4% most of the residents (74%) receiving psychotropic agents were being prescribed only one category of psychotropic drug. Duration of treatment could not be reliably assessed. Psychotropic drug use in geriatric institutions is discussed and studies from different countries are briefly reviewed. Evidence from the literature and the present study suggests that, at least in some institutions, psychotropic drug use is inappropriate.

AbstractOne hundred and eleven patients admitted to an acute medical geriatric assessment unit were screened for depression using the Geriatric Depression Scale questionnaire. All patients with high scores were further assessed by a consultant psychiatrist in a semistructured interview. Of the patients screened, 23.4% had depression of a clinically significant degree and 13.5% were judged to need specific antidepressant medication. 10.8% of the population had depressive illness superimposed on organic brain disease.

AbstractWe report a 3.7 year follow‐up study carried out on 42 patients with an original diagnosis of late paraphrenia who had had a CT scan and simple tests of cognition in addition to an assessment by means of the Geriatric Mental State Schedule. Mortality was found to be no different than in a control group. Patients showed improvement in target symptoms but exhibited a good deal of residual morbidity, particularly of motor and cognitive function. Cognitive performance, which was previously mildly impaired, showed some futher deterioration, usually falling short of clear‐cut dementia. Ventricular size on the CT scan was not a predictor of outcome. The article highlights the organic substrate of late paraphrenia but suggests that the cerebral changes are relatively subtle and slowly progress

AbstractThe cortical appearance on CT scan in patients suffering from late paraphrenia was assessed compared to that of age‐matched normal controls. AbstractA retrospective study of treatment and outcome is described in 59 consecutive referrals to a catchment psychogeriatric service meeting ICD9 criteria for manic depressive illness or depressive neurosis. Of 22 who failed to respond to tricyclic antidepressants, nine (of whom four had also failed to respond to ECT) were treated by lithium augmentation. Systematic comparisons between lithium‐treated subjects, tricyclic responders and others failing to respond to tricyclics revealed no significant demographic differences. Lithium augmentation was successful in 6/9 subjects. Two ‘lithium failures’ were treated with tranylcypromine to good effect. At follow‐up (median six months, range 3–20 months) 7/9 subjects in the lithium‐treated group were well. This was similar to the follow‐up status in tricyclic responders and significantly better than outcome in the other tricyclic non‐responders. AbstractThe study investigated the relationship between social and formal support in promoting the psychological adjustment of spouse caregivers of dementia sufferers. It was found that the quality of social support was negatively correlated with the degree of perceived strain and the level of depression. This result suggests that social support reduces the caregiver's strain or depression. In contrast, formal support was positively correlated with strain, but not with depression, indicating that in the present sample, increased formal support may be reactive to strain in the caregiver. The implications of the findings for the community care of dementia sufferers are discussed.
