SYNOPSISThe nervus intermedius (NI) appears to be the main conduit of cluster headache (CH). Parasympatheticimpulses via the NI account for the associated symptoms of ipsilateral tearing and redness of the eye,congestion of the nose and rhinorrhea. Vasodilatory stimuli to the internal carotid artery may result inpartial Horner's syndrome and set off a cascade of events causing pain, similar to the changes in theexternal carotid arterial tree during migraine. The pain of CH may also be carried by afferent impulsesaccompanying motor fibers of the deep facial muscles.The central nervous system (CNS) mechanisms responsible for the circannual recurrence of CH may beexcitatory with parasympathetic impulses channeled through the NI. Conversely, inhibitory CNSmechanisms may be invoked to explain long remissions, if CH were to be due to hypersensitivity of the NIas caused by the subtle injury of a cross compressing blood vessel.

SYNOPSISSince in migraineurs a change in dopaminergic tone has been hypothesized and since in man prolactin secretionby the lactotroph cells is controlled by hypothalamic inhibitory factors (of which dopamine is the best known), wedecided to investigate plasma prolactin (PRL) response to L‐Dopa in a group of migraineurs.We tested 5 women suffering from common migraine and 5 healthy women as controls.A different behaviour of PRL secretion was observed in the two groups. In migraineurs the inhibition of PRLsecretion was less marked than in normals, with a statistically significant difference at 30 and 60 minutes.This difference might suggest a reduced responsiveness of the pituitary lactotroph cells to the action ofdopaminergic agents. SYNOPSISTen women with histories of migraine attacks and with diagnosis of classic or common migraineconcluded a double‐blind, placebo controlled, cross‐over study to assess the effectiveness of syntheticsalmon calcitonin 100 MRC U. i.m. a day in preventing migraine attacks. After a two‐month run‐in phase thewomen were randomly allocated to one of the following two‐month sequences: placebo‐calcitonin orcalcitonin‐placebo. The patients filled in every day a standard card with number, duration and severity ofattacks. The mean migraine frequency per month on calcitonin was 3.7±0.7 (s.e.m.) and on placebo 7.7±1.3(r<0.01). The ratio of total score of headache intensity to the number of days of observation on calcitoninwas 0.18±0.03 and on placebo was 0.30±0.04 (r<0.01). It is concluded that calcitonin is effective forprophylactic treatment of migraine and should be useful when other first‐choice treatments

SYNOPSISA case of transient global amnesia in a nine year old child with a history of recurrent episodes of vertigoand a family history of migraine headache is described. The possibility that acute confusional state inchildren, presenting as transient global amnesia may represent a variant of basilar migraine is discussed.

SYNOPSISIn a patient suffering from classical migraine, an unenhanced CT scan done a few hours after a recentepisode of headache revealed a low‐density area in the right occipital region. On contrast enhancement, apeculiar high‐density lesion surrounded by a large low‐density area was seen in the same region. This CTscan abnormality totally disappeared spontaneously within two months. Such lesions have been seen inpatients suffering from epilepsy. The possible pathogenetic mechanism is a localized disruption in theblood‐brain barrier during the phase of vasoconstriction. Such a CT scan abnormality in patients sufferingfrom migraine has not been reported

SYNOPSIS“Cold patches” in the external carotid territory previously have been described to have been associatedwith vascular‐related headaches. This study utilizes electronic thermography to examine the relationship.A sample of 275 patients diagnosed as having: vascular, scalp muscle contraction, cluster, post‐traumaticor psychogenic headaches, as well as 45 headache‐free individuals, served as subjects. Each subject wasadministered electronic thermography to determine if “cold patches” were present in the external carotidterritory. A “cold patch” was defined as a region more than 0.5°C cooler than the surrounding area.Chi Square analysis determined that vascular headache patients displayed a significantly greater number(p<.001) of “cold patches” than did normal individuals, or patients having scalp muscle contraction,psychogenic, or post‐traumatic type I headaches. Mixed headaches involving a vascular component alsodisplayed a significantly greater number (p<.001) of “cold patches,” as did cluster disorders (p<.001), andpost‐traumatic type III headaches (p<.001).This evidence supports a valid relationship between "cold patches" and vascular‐related headaches. SYNOPSISTwo children with severe transient focal headaches are presented. Complete relief was obtained aftercyproheptadine treatment. A characteristic feature of these cases was focal persistent irritative EEGdisturbances. All the other diagnostic examinations were normal. The possibilities of migraine alone, or ofheadache as an equivalent of epileptic fit, are discussed separately with a review of the relevant literature.Finally, the probability of an underlying autonomic epileptic phenomenon causing a classic migraineattack is raised.

SYNOPSISFifteen families including 35 patients with chronic headache showing electroencephalographic (EEG)abnormalities and dysautonomia are reported on. Nine families including 18 patients with chronicheadache, without EEG abnormalities, served as controls. In the patients with EEG abnormalities, sixpatients (17.1%) suffered from common migraine, 28 (80.0%) from muscle contraction headache and one(2.9%) from combined headache. All the patients had 6‐7Hz EEG abnormalities such as 6‐7Hz slow waves,6‐7HZ spike&wave complexes and 6‐7Hz positive spikes. Most of the patients (77.1%) manifesteddysautonomic symptoms. These symptoms were observed in only 16.7 per cent of the patients with afamily history of headache, without EEG abnormalities. SYNOPSISThe present study examined psychopathology in 64 volunteers for an on‐going headache research project.Subjects (19 to 58 years old) who responded immediately to newspaper articles requesting volunteersdiffered in levels of psychopathology from those who responded later. It was further noted that thedifferences observed occurred on those MMPI scales found to discriminate among headache diagnosticgroups (Scales 2, 7, and 8).
