SYNOPSISThese experiments investigate thermographic patterns in the posterior cervical/thoracic (PCT) region of 530headache patients and 30 headache/injury‐free volunteers. The study examines: The longitudinal persistence ofProximal and Distal patterns; three distinct midline patterns (PCT I, II, and III); and their correlation with diagnosis,injury, and pain.Twenty‐four (80%) of 30 randomly selected subjects displayed unchanged Proximal patterns at the meanobservation period of 5.5 months. PCT pattern fluctuations occurred in 13/30 (43.3%) subjects. The distinctivenessof each subject's Proximal and Distal patterns was verified by blind calling of thermogram pairs. Patternpersistence was validated with alcohol spray‐Patterns were identical regardless of using a 0.5°C or 1.0°Ctemperature setting. Temperature settings of 1.0°C yielded more distinct Proximal and Distal patterns.Chi square analysis determined that there was no significant difference in the number of PCT III patterns in theexperimental or control groups.In conclusion, it appears that Proximal and Distal Patterns may be consistent over time and individually unique,but that PCT patterns fluctuate and, therefore, do not correlate with chronic h

SYNOPSISPersonality and behavioural features in pediatric migraine were investigated in a comparison study of 20boys and girls with migraine, matched for age and sex with a “pain” control group of 20 children withmusculoskeletal pain, and with a “no‐pain” control group of 20 children. When the amount of pain experiencedby children was controlled, the only discriminating variable was that of somatic complaints which includedvomiting, nausea, and perceptual disturbances, all migraine‐related phenomena. The inclusion of the “pain” control group in this investigation yielded results which indicated that the behavioural and personality featuresthought to be characteristic of childhood migraine are common to a chronic pain disorder and in fact, themanifestation of many of these features correlate directly with the amount of pain experienced. It is suggestedthat the personality and behavioural characteristics evident in many children with migraine may result from therecurrent chronic pain episodes rather than in some way being causat

SYNOPSISA woman developed intractable facial pain following the use of N2 paste for endodontic filling. Radiographsrevealed a radiopaque body adjacent to the site of the nerve supplying the painful area, but removal of thisaccumulation of N2 paste failed to relieve the pain, presumably because of leaching of the components of the N2paste into the tissues. Ultimately, nerve avulsion had to be performed to relieve the pain.

SYNOPSISMethysergide is the drug of choice for the prophylaxis of the crises in Episodic Cluster Headache (ECH). Thissubstance is a partial antagonist of both serotonin and histamine. Nevertheless, the classical anti‐histamine drugsdo not have any therapeutic effect in this disease. The etiological role of serotonin has never been taken intoconsideration, because the plasma levels of this mediator have never been found increased. This is not surprising,since serotonin is rapidly degraded in the circulation or immediately bound by circulating platelets. Therefore, apossible role of serotonin cannot be excluded. Serotonin is capable of modulatingin vitrothe motility of humanmonocytes. Our previous studies showed an increase in the number of circulating monocytes in ECH. Therefore,we examined thein vitrointeractions between circulating cells from ECH patients and serotonin.21 ECH patients were studied, in and out of the crisis period. 5 adults were used as normal controls. Circulatingmononuclear cells were tested for their ability to specifically bind 5‐HT, using ( 3 H) labeled 5‐HT.Normal human mononuclear cells bind serotonin in a saturable and specific fashion, and most of this binding isthrough a high affinity site (Kd = 100nM). Conversely, cells from ECH patients lack this high affinity binding siteand only display binding through the low affinity site. The lack of this high affinity binding can be demonstrated inthe patients during the crisis as well as out of it.A possible explanation for the loss of this high affinity binding site could be that the cells of the patients havebeen previously exposedin vivoto high concentrations of serotonin, probably generated during the crisis p

摘要:
SYNOPSISAuditory brainstem evoked potentials (ABEP's) using the routine 10 per second click rate, and also a 55 persecond stimulus rate, were studied in 17 migraine patients between and during attacks, and in 20 normal subjects.Only in the "migraine during attack" group did the increased stimulus rate change the ABEP's. Similar changeswith increased stimulus rates may be seen in situations of ischemic synaptic dysfunction. Thus, our data supporta vascular pathogenesis of migraine.

SYNOPSISThe subpopulations of T‐lymphocytes and monocytes were studied in 21 Episodic Cluster Headache (ECH) patients, both in and out of the crisis. The following MoAb of the Becton‐Dickinson series were used: Leu1 (pan T cells), Leu2a (suppressor/cytotoxic cells), Leu3a (helper cells), Leu7 (NK cells) and LeuM3 (monocytes). All the subjects were typed for the HLA‐A,B,C and DR antigens of the Human Major Histocompatibility Complex. 10 patients out of 21 were DR5+. Thus, two groups could be distinguished: DR5 + patients and DR5‐ patients. We correlated the variations of the leukocyte subpopulations in and out of the crises with the two groups, since HLA‐DR antigens are related to the immune response.A significant increase of the LeuM3 + cells during the crisis was observed in DR5‐ group (26.2 ± 7.7 vs 18.3 ± 7.1; p<0.05). In the DR5 + group there was an increase of the Leu7 + cells during the crisis (18.1 ± 6.7 vs 12.7 ± 4.9; p<0.05) whereas outside of the crisis the LeuM3 + subpopulation increased (21.9 ± 7.6 vs 18.3 ± 7.1; p<0.02) in the same group. The observed values of the leukocyte subpopulations studied (difference between the data of crisis and out of it) in the two groups were not significantly different. These data confirm our previous studies on Leu7+ and LeuM3 + subsets in ECH patients. Moreover, they allow new speculation about a possible role of Class II antigen

SYNOPSISIn twenty‐one migraine patients, cardiovascular reflex responses and noradrenaline (NA) levels were measured before and after four weeks of calcium antagonist nimodipine medication. Nineteen healthy subjects served as a control group for cardiovascular reflex responses and ten of them as a control group for noradrenaline levels.Neither the cardiovascular reflexes nor the noradrenaline levels differed significantly when the values before and after four weeks of nimodipine medication were compared with each other.The mean pulse rate reaction connected with both normal and deep breathing differed significantly when the results of migraine patients and controls were compared with each other, this result suggesting subtle parasympathetic hypofunction in migraine patients. When the mean Valsalva ratios and the mean postural pulse rate reaction ratios were compared, the ratios in migraine patients were significantly lower than in control subjects (p<0.025), suggesting sympathetic hypofunction in migraine patients. The mean diastolic blood pressure rise in migraineurs in the isometric work test was 15.2 ± 9.4 and in controls 24.0 ± 9.4 (p<0.001). The baseline levels of the NA determinations did not differ when the values of migraineurs and controls were compared with each other; but after 3 minutes of standing, the rise in NA levels in migraine patients was significantly lower than in controls (p<0.05). These results suggest sympathetic hypofunction in migraine

SYNOPSISPressure‐pain threshold was assessed in the forehead, temples, occiput and neck of 102 patients with migraine or tension headache, and in 35 nonheadache control subjects of similar age and sex distribution. Pain sensitivity to the application of a weighted blade to the fingers was also assessed. Tenderness was greater in patients than in controls, particularly at the site of migraine or tension headache, and to a lesser extent in other areas of the scalp. Scalp tenderness persisted for several days after headache had subsided. The threshold for pain in the fingers was greater during headache than during the headache‐free interval but did not differ from pain threshold in control subjects. Pain in the fingers increased more rapidly in patients with episodically‐recurring tension headaches than in other patient categories or control subjects. These findings demonstrate that scalp tenderness is not due to a general increase in sensitivity to pain during migraine.
