SYNOPSISAny effects of lithium carbonate (oral dosage 900 mg/day) on measurements of cerebral hemodynamics were examined in 10 carefully selected patients meeting criteria of chronic cluster headache, i.e., headache‐free intervals were less than 3 weeks in duration per year. Cerebral blood flow (CBF) was measured by the133Xe inhalation method before and 2‐4 weeks after (mean 3 weeks) standard recommended lithium carbonate administration, as a dose of 300 mgs give orally three times per day (total 900 mg/24 hours).Cerebral vasoconstrictor responsiveness was tested by repeated CBF measurements during serial inhalations of 100% O2followed by 5% CO2in air mixtures. Mean values for hemispheric gray matter flow were 70.9 ml/100g brain/min measured over the side of the cluster headaches and 70.0 ml over the contralateral side prior to lithium and 68.0 ml for sides of the head after lithium treatment. These differences before and after treatment were not significant. Cerebral vasodilator responsiveness to 5% CO2was expressed as per cent gray matter flow change per mmHg change of measured PECO2. Compared to values measured in normal age‐matched controls, CO2responsiveness was diminished bilaterally, both prior to and during lithium administration and there were no significant changes induced after treatment with the drug. SYNOPSISA four‐year retrospective study was completed with 693 headache patients who had completed a specific biofeedback training program. Patients 18 years of age and over were also given a Minnesota Multiphasic Personality Inventory to determine psychopathology, depression and anxiety levels, difficulty to motivate, and dependency. The purpose was to examine the long‐term effects of biofeedback training on headache problems. Patients seen between January 1977 and December 1980, ages 6 through 72, with migraine, muscle contraction, mixed migraine and muscle contraction, and cluster headache were mailed an invisibly‐coded 20‐item questionnaire.The effect of such variables as age at time of training, sex, headache diagnosis, elapsed time since training, periodic follow‐up (number of sessions), temperature training with instrumentation reinforcement and relaxation homework, the warm, supportive clinician/therapist relationship with positive expectations for the patient's success, and personality and psychological factors are predictive of success in the application of biofeedback to headache is

SYNOPSISA retrospective study of strokes due to ischemic cerebrovascular disease in young adults was undertaken, and the incidence of migraine as an etiologic factor was studied. Within the last five years, fifteen patients between the ages of twenty and forty were admitted to the USAF Medical Center Wright‐Patterson with the diagnosis of stroke due to ischemic cerebrovascular disease. These patients underwent extensive evaluations including CT scan and/or cerebral angiography. In four patients (27%), there was a previously documented history of classic migraine. Three patients experienced migraine attacks at the time of their stroke. The fourth had an antecedent history of classic migraine with basilar artery migraine symptoms, and suffered a lateral medullary syndrome. The remaining data are as follows: 3 patients (20%) had valvular heart disease, 1 patient had sickle cell anemia, 1 patient had a dural sinus thrombosis following infection and 6 patients (40%) had no discernible etiology. SYNOPSISWe have studied a 24‐year‐old woman followed over a nine year period for episodes of migrainous aura sometimes preceding convulsive seizures and associated with persistent and fluctuant focal and paroxysmal E.E.G. abnormalities: the latter appeared on most recordings and involved the left occipital region. All others investigations remained normal and the clinical course was benign. This case resembles the four cases reported by Camfield et al and an additional case reported by Panayiotopoulos. However our case shows that this syndrome may occur in adults (all the previously reported cases concerned adolescents); moreover our observation shows that convulsive seizures may be or not preceded by migrainous aura, that E.E.G. abnormalities may fluctuate when observed during a long term period (nine years); finally, such as in one case reported by Camfield et al the E.E.G. abnormalities were fo

SYNOPSISThe purpose of the present study is to show anomalies of the plasma cyclic nucleotide responses of patients with migraine to intramuscular injections of methacholine (80 μg/kg body wt) and epinephrine (0.4 mg/person). The methacholine‐ or epinephrine‐induced increases in plasma cyclic GMP were much larger in patients with migraine than in controls. In the case of methacholine injection, there was also a small but significant rise of plasma cyclic AMP in patients with migraine, while no change was observed in controls. On the other hand, there was no significant difference in the epinephrine‐induced increase in plasma cyclic AMP between patients with migraine and controls. SYNOPSISThe incidence of smoking in a group of male cluster headache patients (95%) was significantly (P<0.001) greater than in a matched group of male migrainous patients (26%). Both groups had significantly lower platelet monoamine oxidase activity than controls. This finding may be connected genetically with certain personality characteristics, including predisposition to smoke. It is possible that migrainous patients smoke less than those with cluster headache because the process of smoking has a greater tendency to provoke their headache attacks.

SYNOPSISTheuseof acomputerfor the diagnosis and treatment of headache has been limited; however, studies havesuggestedits feasibility. In the severe chronic headache sufferer there is difficulty in obtaining and assimilating the diagnostic data. This report describes the first phase of a three‐phase project in implementation of a computer as an aid in actual diagnosis and treatment planning for the chronic headache patient. Input into the computer is accomplished with a patient and a physician questionnaire. The computer sorts, analyzes and compares the data with known diagnostic and treatment criteria and weights the relationships numerically. The results are printed in reports including diagnostic impression and differential diagnosis based on the total value accumulated toward those diagnoses. In addition, information is printed to provide physician and patient information specific to the headache diagnoses. A recommended treatment protocol is given for each headache type. SYNOPSISTwo populations of patients, one with common migraine and the other with mitral valve prolapse (MVP), were analyzed to define the relationship between these conditions. Of 100 consecutive patients with migraine, 20 had MVP; of 100 consecutive patients with MVP, 51 suffered from migraine headache. As for features of migraine are concerned, the patients with MVP‐migraine association did not significantly differ from the patients with migraine alone. SYNOPSISRecent attempts to identify EMG elevations in patients with tension headaches have produced contradictory results. In the present case study, a 36‐year‐old male with a 17 year history of tension headaches was treated with trapezius EMG biofeedback while walking on a treadmill. The results demonstrated that: (1) EMG elevations could be identified during dynamic movement but not while in a static position and (2) EMG biofeedback during dynamic movement was effective in reducing EMG levels and headache activity. The results are discussed in terms of the potential importance of assessing and modifying patterns of elevated EMG activity which contribute to headaches during naturalistic activities such as walk
