AbstractThe practice stories of 30 expert home visiting public health nurses were recorded and interpreted qualitatively, using constant comparative analysis. Three family caregiving competencies, which provide the essential preliminary groundwork to enable family self‐help, were identified: locating the family, which involves tracking down highly unstable families; building trust, which involves developing rapport with clients who have seldom experienced trust; and building strength, which occurs with those who have neither apparent strengths nor belief in their own capacity. AbstractOlder adults' use of over‐the‐counter (OTC) medications is usually a self‐initiated behavior that may have important health consequences. A sample of 186 adults age 65 years and over was interviewed to describe their use of 16 OTC substances and their management of the most commonly used OTC medications. Subjects reported using almost twice as many OTC as prescription medications. A structured interview format elicited subjects' self‐reports of decisions, judgments, and actions used to manage aspirins, laxatives, antacids, and vitamins. Most reported using the specific OTC preparations often. Health care providers were common but not exclusive sources of information about these products; few subjects acknowledged being influenced by advertisements. Although subjects' reported dosages and scheduling of agents were judged appropriate by nurses, some overuse of laxatives was noted, together with some continuation of laxative and antacid administration for excessive lengths of time for persisting symptoms. Few subjects identified precautions associated with the OTC medications. Questions were raised about older adults' perceptions regarding vitamin use for particular symptoms. AbstractSince the majority of cancer chemotherapy is given on an outpatient basis, it is important for nurses in the community to understand the concerns that patients and family members experience during the treatment and how they manage these concerns. The 64 adult patients were diagnosed with selected solid tumors or lymphoma and were initiating a course of chemotherapy. Sixty‐nine family members participated as well. Each participant kept a log in which to record concerns and strategies taken to manage them. Patients reported fewer concerns than their families, but both had similar concerns and rated them as high in intensity. AbstractCommunity health nursing leaders recommend including epidemiology and biostatistics in baccalaureate curricula but do not define essential content. Topic coverage and emphasis vary across schools; students differ in academic achievement and in readiness for an epidemiologic approach to nursing. A review of textbooks showed wide variance in topic presentation. A survey of baccalaureate community health nursing faculty in a southeastern state uncovered a core of epidemiology course work comparable to that in graduate programs. Biostatistical content was sparse, and participatory learning strategies were seldom noted. Definition of essential introductory epidemiology and biostatistics content and greater linkage between knowledge and application is necessary to show their congruence with community health nursing practice, and enable differentiation between the level and scope of learning appropriate for undergraduate and graduate education.

AbstractPublic health nurses (PHNs) have been attached to family doctors' offices as one mode of organizing the delivery of nursing services in the community. However, different nurses function in different roles on a continuum from gener‐alist to liaison. A descriptive study was undertaken to determine the most effective role for PHNs attached to family physicians' offices. Outcomes for both physicians and nurses were satisfaction with the role of the PHN, appropriateness of referrals, and accessibility for consultation. These outcomes were compared relative to the rating of PHNs of their role. A cohort of physicians without attachment PHNs were also surveyed to compare responses on the same criteria. The majority of nurses described their role generalist. This role was related to greater satisfaction for nurses and the physicians with whom they worked. Satisfaction with PHN service, ease of arranging consultation with PHN, and appropriateness of referrals from PHNs was greater for physicians who had the attachment than those who used the usual servic
