This paper considers stress in work with mentally handicapped people in hospital. Staff at Meanwood Park Hospital, were invited to contribute their thoughts on this subject. The significant points to emerge were: the nature of stress; the problems of uncertainty in roles; less structured hospital régimes; increasing expectations but inadequate resources; lack of esteem; difficulties with parents and relatives; interprofessional tensions; and dichotomies in attitudes and philosophies. In 1974 L. G. Stewart wrote a paper on the problems and needs of severely handicapped deaf persons and suggested programmes that were needed to meet these needs. He entitled his paper “We have met the enemy and he is us”. He concluded that we have not met the needs of severely handicapped deaf persons, largely because we have not tried hard enough. Though Stewart was writing solely about deafness, this situation can be said to be true of sensory handicap in general. Kropka and Williams (1980) wrote that research into the blind and partially sighted had brought to light hitherto unsuspected service deficiencies for this “hidden” minority, as well as for the hearing impaired minority investigated earlier (Kropka, 1979). The Warrington Community Mental Handicap Team has been functioning for 2 1/2 years, and is the only Community Team in Cheshire actually based in the community. As core members, we have close links with the Psychology Department of the local mental handicap hospital.We describe the implementation of a toileting programme with a severely mentally handicapped 17‐year‐old girl. Several attempts to solve the problem had been made previously and we were trying to eliminate a well‐established pattern of behaviour. We were especially anxious to succeed where others had failed. Four mentally handicapped adults from an ATC were systematically trained (using role‐play and other techniques) to travel by bus, go shopping, and use cafés at a local shopping complex. It was hypothesised that their general social interaction skills would improve as a consequence of teaching them these three specific self‐help or “community living” skills (Marholinet al., 1979).Pre‐ and post‐measurements on theAAMD Adaptive Behavior Scales, and on an observational scale (TAF) devised by the author, showed that this prediction was fulfilled. There were also clear improvements in the three taught skills although these were not always statistically significant.
