Studies are reviewed which contribute information on the costs and benefits of different forms of residential service for people with severe mental handicaps including ordinary housing, larger community services and traditional hospitals. Costs do vary but variation is not always easy to explain, particularly within services of the same type. Moreover, the common expectation that there are diseconomies arising in small scale provision is not borne out in general. Research of service quality has shown better outcomes associated with ordinary housing than with larger and more traditional provision, covering such issues as developmental growth, engagement in community and domestic daily living activities, social contact, quality of the environment, choice and variety of activity. The cost effectiveness advantage of ordinary housing services over traditional hospitals can be concluded. However, this conclusion needs to be qualified by a degree of caution. Housing services have continued to decrease in size and decisions concerning the level of staffing have considerable implications for costs. In addition, more attention needs to be given to ensure that staff input results in outcomes for service users.

This article argues for an empathetic, holistic approach to task analysis skills, formerly perceived as components of behavioural teaching approaches. It suggests that analysis of behaviours and contexts may lead to more appropriate presentation of information to clients with multiple disabilities. The Martian dinner party game is a training exercise supporting the practice of analytical skills in a context allowing discussion of factors affecting their use.

This is the first of two papers focusing upon a new approach to staff development provision within Basildon and Thurrock Health Authority's service for people with learning disabilities. The ‘Education and Training for Change’ programme was set up in January 1990 on a contractual basis between the learning disabilities service (the purchaser) and a local HE College (the provider) which has since become a Polytechnic. Both the papers reflect on different aspects of the programme during its first eighteen months of operation, and attempt to draw some preliminary conclusions that may be of interest to other services.This paper addresses the joint nature of the programme and the particular model of provision adopted, where the staff are ‘in, but not of the service’ i.e. sited in, but not employed or managed by the service. Drinking may be perceived as a highly valued activity by people with learning disabilities and some may, therefore, be prone to drinking to excess without taking the likely negative consequences into account. Alcohol interventions are important and the issue of how these might best be used with this group requires attention. A brief intervention is described in this paper which includes the use of a video‐taped problem drinker to whom clients were asked to give advice. An evaluation, which involved asking clients to specify how they would reduce their drinking, lent some support to the effectiveness of the video‐tape as a teaching method. Observing an infant increases our understanding of non‐verbal behaviours, interactions, emotional development and emerging relationships. An observational component is now included in the one year course at the Tavistock Clinic on psychodynamic work and mental handicap. This article describes my own personal experience in the hope that it will stimulate interest in such an approach. Microcomputers have secured for themselves an important place in the current services for people with learning disabilities. They are seen as prestigious and interesting by clients and staff, if somewhat terrifying at first, and a door into an area for learning new skills in creative ways.
