摘要:
In this country, and elsewhere, there has been a marked shift towards community care and away from institutional care for people with mental handicaps as well as those who are mentally ill. Many aspects of these changes are promising for the people concerned, but the transition also involves some difficulties and challenges for them as well as for health and social services. The article presents one man's experience of life in hospital and discusses the need to ensure that adequate support services are provided for people like him, to maintain them satisfactorily in their new way of life.

This paper draws on the findings of a two‐part study of 55 people discharged from three mental handicap hospitals during the decade 1971–81 to live more independently in the community. Part one, a retrospective study of hospital records, provided data on their lives before they moved. Part two was a follow‐up study of the remaining 50 people living in community settingsaftertheir move. A detailed account of their current life styles was compiled from data from social services records, interviews with social workers, and interviews with the people themselves.Hospital records varied in their completeness but they did describe the people's personalities (personal qualities and motivation), close friendships, outside social contacts, and daytime activities. This article is written (like that of Carpenter, 1984) from my personal experience, in two schools for children with severe learning difficulties, of coordinating a Staff Curriculum Development Group using theSkills Analysis Model (SAM)(Gardner, Murphy, and Crawford, 1983). Little has been written about adults with mental handicaps who can read. A surprising number were found in institutions in South‐East London (68 who were at least semi‐literate, of whom 11 were literate). No explanation of this ability, associated with IQ's as low as 40, was found in the literature or in this study. Comprehension scores were on average more than a year lower than Accuracy scores, giving support for the notion of “barking at print”. The Jay Project was set up to devise a method for assessing the needs of individuals with mental handicaps, with particular reference to the services that they were then receiving and to identify future service needs. It presented service needs from the point of view of the individuals, rather than trying to fit them into current service provision. Assessments covered all the major areas of life. Findings were presented to a Steering Group whose members were drawn from all the relevant local statutory and voluntary agencies.
