SummaryWe have designed a human basophil degranulation test based on a one‐step method of basophil staining after exposure to a specific allergen. This very simple test is now commercially available as a ready‐to‐use kit. It explores the allergic sensitivity at the cellular level, probably through IgE‐dependent sensilization of the basophil. It positively correlates with histamine release and radioallergosorbent test (RAST). It detects sensitivity to common allergens and to chemical and food products as well. In its present state, the test provides a simple, inexpensive specific means forin vitrodiagnosis of allergic diseases, Recently, it has been further developed to be performed on the automatic Hemalog D Technicon

SummaryThe infiltration by basophils into delayed hypersensitivity skin lest sites was examined in patients with atopic dermatitis, contact dermatitis and in normal healthy persons.Atopic dermatitis patients, with large amounts of IgE, injected intradermally with staphylococcal antigens showed reactions that were more transient and erythemaious than those of normal persons. On hislological examination there were numerous, degranulating basophils among the perivascular mononuclear cells. Normal persons, with small amounts of IgE showed typical mononuclear cell infiltration and few basophils. Patients with contact dermatitis, one with much IgE, responded to patch tests to potassium dichromate or to nickel sulphate by delayed type reactions, but on histology, two of the four patients showed a significant infiltration by basophils.It is considered that the erythematous response to the antigen in atopic dermatitis patients is related to the basophil infiltration.

SummaryQuantification of human basophil degranulation induced by anti‐IgE, anti‐IgG4, and by ionophore was performed using a flow‐cytometric system. It was shown that these antibodies and ionophore can degranulate basophils in a dose‐dependent manner, and that there is a wide variation in the response of basophils obtained from different individuals to these stimuli. A significant correlation was observed between the degree of degranulation induced by anti‐IgE and anti‐IgG4, while this was not the case for anti‐IgE and ionophore. SummaryPeripheral blood basophils of thirty‐eight patients with bronchial asthma were examined for their reactivity to anti‐IgE and anti‐IgG antisera. Basophils of patients with serum IgE levels of more than 1001 i.u./ml reacted strongly to anti‐IgE. Basophils of patients with serum IgE levels of less than 100 i.u./ml had a tendency to react more strongly to anti‐IgG. An index (basophil ratio) was devised to compare the patient basophil reactivity to anti‐IgE and anti‐IgG. This basophil ratio was lower (IgE dominant) in the atopic cases which usually exhibited a high serum IgE level. Most cases with low serum IgE exhibited a high basophil ratio (IgG dominant). SummaryThe migration velocity and morphology of basophilsin vitrowere examined after the addition of anti‐immunoglobulin (anti‐lgE or anti‐IgG). In atopic asthma patients with high serum IgE levels (more than 1000 i.u./ml) basophils showed increased migration velocity and showed pear‐shaped and palmate processes after the addition of anti‐lgE, but on the addition of anti‐IgG, these basophils did not show increased migration velocity or morphological changes. SummaryA patient with severe asthma induced by the inhalation of plexiglas dusts is reported. Inhalation testing against various industrial products to which he was exposed at work, showed a specific and reproducible asthmatic reaction to plexiglas dusts. Complete pulmonary investigations after positive challenge excluded associated alveolitis and demonstrated an immediate and a late obstructive response. The patient was not atopic and his clinical history did not suggest occupational disease due to chronic exposure. Reduction of the plexiglas dust exposure resulted in progressive improvement.

SummaryOxatomide is an H1, receptor antagonist, which also inhibits antigen‐induced mediator release from sensitized mast cells and basophils. The effects of ingested oxatomide (30 mg/capsule) were compared with those of chlorpheniramine (5 mg/capsule) in a double‐blind non‐crossover study in two groups of twenty‐five subjects with ragweed pollen‐induced hay fever during the pollen season. Oxatomide or chlorpheniramine was started when symptoms became troublesome and was used in a dose of two to four capsules daily according to severity. Other medications were added, in a standardized way, if hay fever symptoms were not controlled by the test capsules. Forty‐one subjects completed the study; nineteen received oxatomide and twenty‐two chlorpheniramine. Mean symptom scores, number of test capsules and number of doses of additional medication were not significantly different between the two groups. Drowsiness was reported by subjects in both groups but was worse in the chlorpheniramine group (P<0.05). SummaryThe effect of three forms of sodium cromoglycate (SCG), 20 mg, on allergen‐induced early asthmatic responses was examined in ten stable asthmatics. Dose response allergen inhalation tests were performed on five occasions at intervals of from 1 to 2 weeks to determine the provocation concentration producing a 20% reduction (PC20 allergen) in FEV1. Placebo was given before the first and the last tests to determine the reproducibility of responses to allergen over the study period; reduced responsiveness was observed in eight of the ten subjects. Major changes in levels‐of specific serum antibodies of the IgE and IgG classes did not serve to explain the changes in bronchial responses although there was a trend which suggested IgG‐related desensitization. The observed changes in bronchial responses and antibody levels illustrate the requirement for tests of reproducibility of responses by the use of placebo controls at the beginning and end of a series of allergen inhalation challenges.SCG as (i) a micronized powder with lactose, (ii) micropellets without lactose, or (iii) an aerosol, were inhaled double‐blind, in random order, 5 min before the additional three allergen inhalation tests. PC20allergen was reduced following SCG in seven subjects; the differences were statistically significant for the group. There was no observed difference in efficacy between the different forms of SCG.
