SummaryA case of pigmented purpuric dermatosis is reported. The patient was a fit young girl subject to recurrent, generalized, transient purpuric capillaritis appearing 36–55 hr after systemic absorption of house‐dust mite antigen. Her serum contained IgG and IgE antibodies to house dust and to mite antigens. Skin from a test site had IgE on the mast cells, and half of the cells showed anaphylactic degranulation. SummaryGroups of atopic and non‐atopic individuals were immunized intranasally with fluid tetanus toxoid, an antigen which normally does not gain access to the nasal cavity. Anti‐tetanus toxoid antibody response was evaluated in nasal wash, sputum and serum specimens by passive haemagglutination.Titres were high in serum and low in exocrine fluids throughout the course of immunization. Mean nasal wash titres rose to a significantly higher level in the normal than in the atopic group. Twelve of twenty normal subjects responded with a greater than three‐fold rise in nasal antibody as opposed to one of nineteen atopic individuals. SummaryIn vitrolymphocyte transformation and the precipitating antibody to house dust and mite (Dermatophagoides pteronyssinus) extract were studied in mite‐ and house‐dust sensitive patients and in non‐atopic individuals. SummaryTwenty‐eight selected patients with pulmonary aspergilloma and many with respiratory symptoms were investigated. Their atopic status was evaluated and immunological reactivity to extracts ofA. fumigatusstudied by means of skin, serological and inhalation challenge tests. The findings suggested that immunological mechanisms may have played a role in producing symptoms in 75% of these patients. Based on these findings, corticosteroids may have a place in treatment of some patients with pulmonary aspergilloma. Care in the use of corticosteroids is recommended because of the prevalence of bacterial infection. Suggestions are offered for selecting patients likely to respond to corticosteroid drug

SummaryEleven asthmatic patients with a history of asthma with or without rhinitis following the ingestion of alcoholic drinks were challenged by oral ingestion of the suspected drink, and some with equivalent amounts of ethyl alcohol. Asthma was provoked in six cases, four having falls in the FEV1of more than 15%. SummaryFive patients with a history of post‐exercise bronchoconstriction and eleven control subjects were exposed to gradually increasing work loads on a bicyle ergometer. The asthmatic patients showed higher blood lactic acid levels at all work loads than the control subjects. In contrast to findings in the controls, the plasma free fatty acid in the asthmatics failed to ‘rebound’ following the cessation of exercise; and in two out of three patients plasma FFA did not rise after epinephrine injection. In addition, the asthmatic patients consumed less oxygen during the exercise than the controls.The results of this study suggest that patients with post‐exercise asthma may have to rely more on carbohydrates as the main source of energy because of the reduced availability of FFA. SummaryAllergen extracts from birch pollen, timothy pollen and horse dandruff were investigated. The influence of storage time, volume of extract in the vial and various additives on the potency of diluted extracts was tested with the radioallergosorbent test (RAST). The potency of diluted pollen extracts was found to decrease more rapidly than the potency of diluted dandruff extracts. The differences in allergen activity seen in RAST between freshly diluted extracts and stored extract dilutions were roughly reproducible in skin tests and histamine release tests from passively sensitized chopped human lung. The higher allergen stability of the dandruff extracts could, at least in part, be explained by the higher protein content of these extracts. An adsorption of allergen to the glass wall was shown to be the most probable cause to the more rapid decrease of allergen activity seen in pollen extracts stored in vials only partly filled. With addition of 0.2% Tween 20 after storage the potency of the extracts became almost equivalent to that of corresponding dilutions stored in filled vials.
