Hospitals face a set of issues critical to strategy formulation that are markedly different than those faced by traditional corporations. Given this, the relevance of current business approaches to strategy formulation that focus exclusively on a narrow set of market-driven factors is questionable. Hospitals, it is argued, must take a more comprehensive view of strategy formulation that goes beyond issues of internal competency and environmental opportunities to incorporate the values of caregivers and the societal responsibilities of the institution as well.

摘要:
Not all rural hospitals are in a depressed financial situation. Many can and have achieved financial performance levels which match their urban counterparts. Cost control is the single most important management strategy which differentiates the successful from the unsuccessful rural hospital. Labor productivity is much higher in the financially successful rural hospital than in the unsuccessful hospitals. Reduced length of stay is also especially critical in the overall cost containment program.

Urban-rural hospital affiliations are an outgrowth of both the external pressures on rural hospitals to survive and the need for urban hospitals to maintain or increase their share of the tertiary referral market. This article discusses the significant role of stakeholders in these affiliations develops a fourfold typology of urban-rural hospital affiliations based on the notions of organizational control and fit, suggests four generic strategies for forming affiliations, and analyzes four actual cases of affiliation.

Of the many characteristics that classify the practice of medicine as a profession, autonomy plays the most important role. There are currently assaults on the profession's ability to select, train, and license its own practitioners and on the medical knowledge base itself, as well as the standards for applying it. This article examines factors contributing to this reduction in autonomy and reviews potential impacts on the profession, patients, payors, health care organizations, and managers.

Health care executives must lead the way to the comeback of “voluntarism” in their field. This will require having four key political habits: (1) acting out of a sense of civic responsibility, (2) avoiding the dangers of faction and “magic bullets”, (3) building alliances by a process of conferring with other relevant leaders and groups, and (4) participating in community activism, i.e., acting on, not reacting to, local problems and pressures.
