This article analyzes the importance and effectiveness of several physician recruitment strategies in 60 shortterm general hospitals in rural Minnesota. The results suggest that rural hospitals should continue to attract physicians with quality facilities and services, increase efforts to facilitate group practice opportunities, and rely less on direct financial incentives.

Management science and operations research (MS/OR) refer to the development of mathematical and computer models to solve complex decision problems. In this day of intense competition among health care providers and pressure from government and third party agencies to become more efficient, administrators must make the most of available resources. MS/OR models can help accomplish this. Since MS/ OR models can be quite complex, this article discusses how administrators can work with analysts to promote the success of the model implementation.

Health systems will profit most under capitation if their vertical integration strategy provides operational stability, a strong primary care physician base, efficient delivery of medical services, and geographic access to physicians. Staff- and equity-based systems best meet these characteristics for success because they have one governance structure and a defined mission statement. Moreover, physician bonds are strong because these systems maximize physicians' income potential and control the revenue stream.

Increased management of physicians is inevitable in today's economically competitive environment. Health maintenance organizations (HMOs) and other managed care settings are challenged by and often confronted with negative perceptions of physicians relating to management and organized practice. Paradoxically, reported experiences of physicians show these negative perceptions to be misconceptions. Managers need to eliminate this perceptual bias through physician education and dialogue.

A 1994 survey of hospital HRMs has revealed that charges of sexual harassment within the health care industry are increasing at an alarming rate. Most charges are filed by women who claim they were victims of “hostile environments.” Nurses levy the largest number of complaints, followed by clerical/ secretarial personnel, technicians, custodial workers, food service personnel, and therapists. Most charges are filed against coworkers. Approximately 10 percent of all charges of sexual harassment in hospitals are brought against physicians.

The Ocean State antitrust case illustrates the operation of the competitive marketplace in health insurance. Blue Cross, the dominant firm in Rhode Island, responded competitively to the entrance of a new competitor, Ocean State, in three ways: 1) a most-favored-nation clause, 2) creation of a PPO offering similar benefits as Ocean State, and 3) an adverse selection policy. These actions are assessed to be legitimate competitive responses and the decisions of the higher courts overturning the jury verdict against Blue Cross are supported.

The modern hospital has evolved through three phases since 1900. Between 1900 and 1950, hospitals were in a charitable era and from 1950 to 1990, they were in a technological era developing a comprehensive range of clinical services. From 1990 to well into the 21st century, hospitals will be in an economic era where decision making will shift from the boardroom to the administrative suite. Clinical and management viewpoints will be blended in order to successfully compete in the new environment.
