In the study on which this article is based, nursing homes with the most improvements in resident outcomes had greater registered nurse (RN) participation in decision making than did homes with the least improvements. The results suggest that nursing homes that want to improve quality can use RN participation to make improvements without significantly increasing costs. Complexity theory served as a framework for the study.

摘要:
This article describes the efforts of a small children's specialty hospital to prepare for a difficult period of restructuring in the marketplace by using the organization's core values as a force for direction, stabilization, and leverage. A best practices model was used to identify values that were a central part of the hospital's cultural heritage but were also critical for ensuring future institutional success. Principles for values definition and implementation provide guidance for other health care institutions.

Many hospitals are actively pursuing strategies that integrate physicians into their management and governance structures. Despite expectations that these strategies improve hospital efficiency, empirical studies have failed to provide consistent evidence that physician involvement in hospital management and governance improves hospital efficiency. This article examines factors that may moderate the relationship between physician participation in hospital management and governance and hospital efficiency.

摘要:
Recent marketplace realities and trends have forced health care institutions to adopt strategic orientations that stress a customer focus. Central to such strategic orientations is the effective utilization of service quality practices and philosophies. Toward that end, this research offers health care institutions an affordable methodology. A real-life application of the rapid assessment methodology (RAM) in a health care operational setting is presented. Finally, a framework to guide the implementation of the RAM methodology is outlined and explored.

A conceptual framework for HMO managers who are evaluating physicians' financial incentives proposes that the incentives vary along five dimensions: (1) the percentage of the physician's income at stake, (2) the organizational level, (3) the synergy between multiple financial incentives, (4) the synergy between financial and nonfinancial incentives, and (5) signaling effects. Managers can apply this framework to assess the intensity of physicians' financial incentives and to devise better incentives.

This article reports on observations of the implementation of a patient-centered care (PCC) work reorganization model in a community hospital setting. Analysis of videotape, direct observation, and interviews with key informants demonstrated the similarities between organizations and families in times of change. We propose a family systems framework for understanding some of the complex relationships and transitional experiences we observed.

This article reviews management literature on health care transformation and describes the processes, including restructuring, job redesign, and downsizing, involved in one academic medical center's experience. The article concludes with lessons learned at each of the stages of the transformation process: planning, implementation, and process continuation. Managerial implications for similar transformation efforts in other health care organizations are suggested.

The acquisition of private medical practices by large health care systems often results in the transition of the physician from self-employment to salaried employee. In order to be successful, new physicians must be socialized into the new environment, requiring both concrete information and cultural orientation. This Mayo Clinic study of recently hired physicians yielded information about both content and delivery mechanisms that will help in planning an integrated system of orientation experiences.
