Life long living addresses the challenges that persons who have experienced a traumatic brain injury face following formal rehabilitation. Each program is unique according to personal abilities and resources, with consumer control essential for program success. Residential placement is not synonymous with lifelong living, and many supports can be developed from existing community services, with specialized programs used as necessary. Because of the length of most programs, funding is often a formidable challenge, and creative approaches are required to moderate costs. The ultimate success of lifelong living programming is measured by the lack of institutionalization that it experiences over time and how it helps individuals improve the control and quality of their lives.

The number of persons who survive a traumatic head injury with substantial functional limitations is rapidly increasing. Their needs for care and assistance and the cost of meeting such needs are substantial, Yet, our society has not determined how responsibility for the lifelong well-being of persons with head injury should be allocated. Responsibility currently is distributed according to a triadic model among the headinjured individual, the individual's family, and society; the family often assumes the brunt of the burden. This article suggests an alternative model in which the resources of the community serve as a supplement to the resources of the individual, the family, and society. It argues that, while society must accept the fallback position, public policy should foster the growth of the currently underdeveloped community sector to better meet the needs of persons with head injury.

Independent living centers (ILCs) are an existing network of human service agencies designed to help people with disabilities live more independently and contribute to their communities. These nonresidential, community-based programs emphasize consumer control over the choice of goals and solutions to problems, use of peer role models, a cross-disability focus, and encouragement of self-help approaches to problem solving. Services and activities often Include housing assistance, skills training, advocacy, peer counseling, attendant services, information and referral, and transportation. ILCs represent a lifelong community resource that is often underutilized by people with traumatic head injuries.

Data from the Kansas City Head Injury Survey indicate that the long-term needs of people who have experienced a traumatic brain injury will vary according to their level of recovery, dependency, living situation, support systems, and resources. Often, the services that are available (as compared to the services that are needed) will further shape a person's destiny. Three cases are presented demonstrating the diversity of people's needs and how these needs can change over time.

Comprehensive, supportive care programs for low-income, traumatically brain-injured adults are in short supply in the United States. To create effective, community-based care alternatives for this population, national advocacy groups, caregivers, families, and concerned citizenry must piece together federal, state, county, municipal, and private funds. Since 1978 Accessible Space, Inc (ASI), a nonprofit organization based in Minneapolis-St. Paul, Minnesota, has been providing such services for persons with traumatic brain injuries and other disabilities, integrating affordable housing with vocational, social, case management, transportation, and other services required for lifelong living. The article reviews the design of the ASI system, its operation, and its funding mechanisms.

Costing long-term living is a four-step process. First, it is necessary to identify the elements of the desired future over time in all their personal and financial complexity. Second, all the related elements of the present state must be understood. Third, it is necessary to think through what achieving the desired future will do for the person and family. Fourth, the “how” of achieving the desired future must be addressed. All this must be based on an objective assessment of net worth and the physical and mental ability of die person or family. Planning must use assumptions about future medical and consumer inflation, future interest rates, future returns on investment, and eligibility for assistance programs.

Rehabilitation of the individual with traumatic brain injury (TBI) requires a systems approach that views the functioning of the individual as a balance among the psychosocial features of the person, the physical status of the body, and the environment in which the person behaves. This balance fluctuates across a lifetime, particularly in the first several years after TBI. The article attempts to describe the systems aspects of rehabilitation of the person with TBI by using both a sickness treatment approach and a health care approach. Suggestions for policy changes at the national level are offered.

The assessment of posttraumatic amnesia (PTA) duration after a closed head injury provides one of the earliest and best predictors of long-term outcome. Traditionally, PTA has been assessed retrospectively, but the numerous difficulties associated with this approach have led to the development of two instruments that track PTA prospectively. The study described in this article compared the results of both of these instruments, the Galveston Orientation and Amnesia Test (GOAT) and the Orientation Group Monitoring System (OGMS), for an inpatient rehabilitation population of 21 severe traumatically brain-injured patients. The study provides construct validity for both instruments and suggests that they have relative advantages depending on the purpose and setting in which they are used.

Capacityandcontrolare presented as two basic dimensions for understanding attentional problems and devising appropriate treatment strategies. A theoretical explanation of this approach is given, together with clarification of how these dimensions relate to other attentional terms and concepts found in the literature. Clinical cases are provided to illustrate the way that the dimensions of capacity and control manifest themselves during cognitive rehabilitation, and practical strategies for dealing with deficits are outlined.
