AbstractA computer based system to follow until death all discharged patients with a malignant diagnosis has been implemented at Roswell Park Memorial Institute. The automated system has relieved the follow‐up clerks of tedious clerical procedures and the human decision‐making that was prevalent in its manual predecessor. The system automatically prepares letters based on the patient's condition and, when the patient expires, a physician letter is generated to obtain details of the death (autopsy, mechanism of death, etc.). All correspondence is stored on magnetic tape for patient as well as system evaluation. Less than one percent of the cases registered has been lost to follow‐up. AbstractBreyere and Williams (1964) have reported partial and complete rejection of skin isografts from leukemic donors transplanted to nonleukemic recipients. The present studies were performed to determine if the same phenomenon could be demonstrated with other leukemia viruses in different inbred mouse strains. Inbred mice which were inoculated with either 334‐C leukemia virus or Friend virus served as donors of skin isografts after development of leukemia. Skin grafts were transplanted to either normal mice or immunized mice which had received injections of 334‐C leukemia cells or Friend virus prior to skin grafting. All mice had control isografts. Results showed nearly 100% survival of control isografts. An almost identical percentage of grafts from 334‐C leukemic donors survived in normal recipients (88.4%) as in immunized donors (87.5%). However, skin from mice with Friend leukemia was rejected in a higher percentage of immunized mice (50%) than non‐immunized mice (30%). No takes of skin from leukemic donors were observed in a mouse strain naturally immune to induction of Friend leukemia. Results in the Friend leukemia system support Breyere's concept of an immunologic mechanism of syngenic skin graft rejection, whereas those in the 334‐C leukemia system do not. AbstractThe metabolic activities of proliferating human tissues which concern nucleic acid synthesis and the cell cycle are being examined by high resolution autoradiography following the incorporation of labeled nucleic acid precursors. The relationships of the kinetics of cellular proliferation are investigated using a method whereby surgical biopsy specimens are labeled with nucleic acid precursorsin vitro, but under conditions in which the incorporation of the label occurs only in the cells which were synthesizing DNA and RNA in the patient, thereby providing a pattern of labeling similar to that obtained byin vivomethods. The tissue turnover rates, potential doubling times, and the temporal patterns of DNA and RNA synthesis in relation to the cell cycle of proliferating cell populations are determined by appropriate techniques using3H‐ and14C‐labeled thymidine and uridine. AbstractA retrospective study of forty‐two of forty‐nine cases of Wilm's tumors treated at the Roswell Park Memorial Institute over the past forty years is presented. Abdominal palpation to date remains the most important finding. Since over 20% demonstrated nonfunction on intravenous pyelogram, preoperative diagnosis and differentiation from neuroblastoma are therefore not always certain. Artereography may be helpful in selected instances. Metastases were present in 29 cases at the time they were first seen at the Institute. Modes of therapy were; nephrectomy, 41 cases; plus radiation therapy, 40 cases; and plus chemotherapy, 27 cases. The relationship of age of onset, stage of tumor, presence of metastasis, mode of therapy, and length of survival is presented. Current therapy includes nephrectomy, radiation therapy, and chemotherapy with actinomycin D or vincristine, or both, and monthly follow‐up is advised. AbstractThe problems associated with the establishment of permanent bile drainage in the presence of inoperable bile duct obstructions are discussed, and the possibility of achieving effective drainage by means of an in‐dwelling transhepatic tube is described. Such a tube may be leftin situfor the remainder of the life of patients suffering from carcinoma or indefinitely in patients suffering from nonmalignant obstructions. AbstractA series of 15 patients with disseminated prostatic carcinoma were treated with open or cryosurgical hypophysectomy, following relapse from conventional forms of hormonal therapy. Six of 15 patients (40%) had objective and subjective signs of clinical and chemical response. To date, however, patients in remission have not demonstrated a significant prolongation in survival. Human growth hormone (HGH) levels were used to test the degree of functional suppression of pituitary function posthypophysectomy. Cryosurgical hypophysectomy, although an admittedly anatomically incomplete procedure, can induce marked reduction in activity of the anterior pituitary as judged by the HGH responses. The levels of plasma growth hormone suggested some residual pituitary tissue (hypofunction) rather than complete ablation. The decreased operative mortality and present results with cryosurgical hypophysectomy for disseminated prostatic carcinoma are encouraging and may permit the application of this form of therapy to a greater number of patients.
