AbstractThis study is a review of 66 patients with a diagnosis of testicular seminoma, treated at the Department of Radiation Oncology, University of Louisville, from 1959 to 1978. Sixty‐four of the patients were diagnosed as pure seminomas and two as spermatocytic type. The age of the patients ranged from 16 to 75 years with a median of 39. Fifty‐four patients (82%) presented with swelling or mass in the testis, 7 patients with pain, 1 patient with hydrocele, and 4 patients had seminoma diagnosed incidentally. The distribution was equal for both right and left sides. There were 39 (59%) and 27 (41%) patients in stages I and II, respectively. All stage I patients received only infradiaphragmatic irradiation by a “hockey stick” field, and the majority of them received a dose of 3,200 – 3,600 rads in 3–4 weeks time. Stage II patients received elective irradiation to the mediastinum and neck region, in addition to the infradiaphragmatic irradiation. The 5‐year actuarial survival rates for stage I and II seminomas are 96% and 92%, respectively. There were no serious complications during follow‐up. AbstractSix cases of choriocarcinoma, 17 of vesicular mole, 20 normal pregnant females in the first trimester of pregnancy, and 20 normal nonpregnant women with previous normal obstetrical history were evaluated quantitatively for cell‐mediated immunity by response to 2:4 dinitrocholoroben‐zene and recall antigens. Serum immunoglobulins were estimated in all cases. Suppression of cell‐mediated immunity was marked in choriocarcinoma as compared to vesicular mole. Serum IgG levels were significantly diminished in cases of vesicular mole, indicating diminished humoral immunity. AbstractThe clinical, light, and ultrastructural features of a myoepithelioma occurring on the hard palate of a 24‐year‐old woman are presented and compared with 41 myoepitheliomas of the head and neck described in the English literature. These 42 tumors (39 benign and 3 malignant) occurred in individuals from 14 to 81 years of age, affected both sexes about equally, and most often presented as a slowly enlarging, asymptomatic mass. The parotid gland and palate were the most common sites of occurrence. The tumors are typically circumscribed and encapsulated, vary from 1 to 5 cm in the greatest dimension, and are composed of spindled and/or plasmacytoid myoepithelial cells. AbstractA review of the most relevant clinical data of patients with primary carcinoma of the gall bladder referred to our hospital over a 16‐year‐period has indicated that this is the third most common and aggressive cancer of the gastrointestinal tract. The disease occurs predominantly in elderly females who often present with extremes of clinical symptoms indicating benign biliary disease on the one hand and incurable malignant disease on the other. Laboratory and roentgenographic data tend to confirm the clinical diagnosis of this almost entirely incurable disease. AbstractThe courses of 208 patients with adenocarcinoma of the pancreas were reviewed. The lesion was located in the head of the pancreas in 142 patients, (68%) and of these, in 22 patients the diagnosis was confirmed histologically at postmortem examination; 21 patients underwent laparotomy and biopsy with a 33% operative mortality and 3.4‐month average survival; 89 patients underwent biliary and/or gastric bypass with a 24% mortality and 4.8‐month average survival; 10 patients underwent pancreaticoduodenectomy with a 20% mortality and 14.6‐month average survival. The lesion was located in the body or tail of the pancreas in 77 patients (32%); and, of these, 15 patients had histologic confirmation of clinical diagnoses at postmortem examination; 19 patients underwent biopsy of extra‐abdominal metasases and survived an average of 1.4 months; 27 patients underwent laparotomy and biopsy with a 26% operative mortality and 3.5‐month average survival; 4 patients underwent gastric and/or biliary bypass with a 50% mortality and 4.5‐month average survival; one patient underwent noncurative distal pancreatectomy and survived 1 month postoperatively. AbstractVentriculoperitoneal shunt metastases from primary intracranial germinomas are extremely rare. To date, only three case reports of this phenomenon have appeared in the literature. We present the case of a 25‐year‐old black male who developed ventriculoperitoneal shunt metastases 10 months following diagnosis of an intracranial germinoma.
