AbstractBased on the Barr body frequency, a total of 285 unselected breast cancer tissues were categorised as negative and positive tumours; incidence of these two groups in our series was 2:1, respectively. When 2‐years' disease‐free interval and 10‐years' survival were considered, it was seen that patients harbouring negative tumours had significantly early recurrence and shorter duration of survival as compared to those having positive tumours. AbstractBreast cancers from 53 patients were explanted in culture, and 39 with two‐year or more follow‐up and documentable tissue forms of cancer in culture (histoculture) are the subject of this report. Clinicopathologic findings have been correlated with dynamic aspects of the cancers in short‐term histocultures, including cell cohesiveness, yield and survival time of the cancer cells in culture, emigration of macrophages from the explants, and the occurrence of special interactions of lymphocytes with cancer cells.The outstanding finding was the association of special lymphocyte‐cancer cell interactions with a more favorable prognosis; six of seven patients (86%) with infiltrating ductal cancer showing lymphocyte congregation, emperiopolesis, or other special activity were tumor‐free survivors. Additional findings were: (1) patients having four or fewer lymph nodes positive for cancer had a survival rate of 50%, but this fell to 20% when emigrating macrophages werenotpresent in the cancer cultures; (2) eight of 12 patients (67%) having discoid histoculture survival time of less than one week were tumor‐free, as opposed to three of eight patients (27%) where tumor survival in vitro was greater than one week. The reverse was true for nondiscoid, poorly cohesive colonies of cancer; (3) the combination oftumor‐negative lymph nodes and low in vitro cancer yieldwas associated with the best patient survival rate, 64%. However, the combination of negative nodes and high in vitro yield was linked to the worst patient survival (20%).Although the overall study does not permit definitive conclusions, there is an undeniable potential for the use of histocultures in the evaluation of human cancers. AbstractTen patients with extremity sarcomas adriamycin 60 mg/M2into the artery supp supplying the area of tumor. There were minimal local side effects consisting of occasional local erythema or slight transitory pain. Nine of these patients had subsequent surgery, and an average 32.86% histologic tumor necrosis was recorded in the peripherally viable areas of seven patients with residual tumor, compared to a 5.71 % necrosis recorded in the biopsy sections (P value<0.01).

AbstractAlthough the therapy of carcinoma of the colon and rectum has been investigated, little progress has been made in prolonging the survival in these patients. Specific active immunotherapy may provide the answer. Previous investigators have shown that colorectal cancer elicits an immune response and may be susceptible to immunologic manipulation. AbstractThe syndrome of hypomagnesemia in patients receiving total parenteral nutrition (TPN) is well known. To determine particular high‐risk groups for the development of this syndrome, 26 consecutive patients on TPN were initially evaluated for serum magnesium (Mg) and followed at regular intervals. Seventeen had a diagnosis of solid tumor or hematologic malignancy (CA); nine had inflammatory bowel disease and/or small bowel fistulae (ID). All met the standard criteria for being malnourished – anergy, low serum albumin, and recent weight loss. During TPN, all patients received an average of 24 mEq of magnesium sulfate per day, and all had satisfactory anabolic response in terms of weight gain and increase in serum albumin. Ten patients had at least one magnesium determination below the lower limits of normal, and four of these developed symptoms of tremor which responded to increased amounts of magnesium in their TPN. Eight of these ten (80%) had a diagnosis of CA, and four of four (100%) of those requiring additional magnesium to alleviate symptoms had CA. None of the patients with ID developed symptomatic hypomagnesemia. We conclude that patients with solid tumor malignancy are more likely to develop hypomagnesemia, possibly because of the increased requirements for magnesium in lymphocytolysis of tumor cells, and they must be carefully monitored to prevent this complicat

AbstractThe development of appropriate animal model systems has been proposed as a means of facilitating the study of human colorectal cancer. This report describes the development and use of a blind‐colorectal pouch in a carcinoma mouse model.The blind‐pouch was prepared in C57BL/6J mice by surgically exteriorizing the descending colon and producing two stomata in the abdominal wall. The proximal stoma served as an end colostomy and the distal stoma created as a mucous fistula. Surgical closure of the anus thus provided a colorectal pouch.In pilot studies it was found that N‐methyl‐N‐nitrosourea (MNU) and radiation together but neither separately produced tumors in the pouch of surviving mice. Further, inoculation of C38 syngeneic tumor cell suspension into the pouch and immediate closure resulted in tumor takes within three to four weeks. AbstractThe human colon carcinoma cell line HT‐29 was adapted to grow in chemically defined medium (CDM). The spent CDM (S‐CDM) was concentrated by Amicon filtration and the crude HT‐29 S‐CDM purified by 40% saturated(NH4)2SO4precipitation. The purified antigen was tested by a micro‐complement fixation (MCF) assay against the sera of cancer patients of various histologic types and against the sera of normal donors. Fifteen of 20 (75%) colon cancer, 16/20 (80%) breast cancer sera, 14/19 (74%) lung cancer sera, and 13/20 (65%) miscellaneous carcinoma sera were positive in the MCF. By contrast, 2/21 (10%) melanoma sera, 7/20 (35%) sarcoma sera, and 2/19 (11 %) normal sera were positive. AbstractTo evaluate the clinical significance of99mTc‐sulfur‐colloid (TcSC) spleen scan findings in patients with malignant melanoma, a retrospective study was undertaken. Eighty‐one patients with histologically proven malignant melanoma who received treatment in Roswell Park during a five‐year period were included in this study.The scans were analyzed for spleen size, differential uptake of the tracer in liver and spleen, and for the presence of metastases in these two organs. These data were compared with stage of disease, survival, and autopsy findings.Significant correlation was found between the splenic size as measured on the scintiscan and at autopsy examination. The spleen size was found to be normal in 92% of the patients in early melanoma. The median survival of patients who had a normal‐sized spleen by scan criteria was found to be longer than those who had splenomegaly. No significant difference in survival was noted between the patients with and without augmented splenic uptake of TcSC. AbstractLeVeen shunts were placed in ten patients bedridden with intractable malignant ascites in order to improve comfort and ambulation. The procedure was done with local anesthesia and mild sedation in all but one patient. Mild prolongations in clotting studies and hypokalemia were noted in several patients, but severe complications were few: a bleeding diathesis in a jaundiced patient, and venous disease in two. Six of the ten had excellent clinical response and were able to undergo therapy impossible in the presence of massive ascites. Two had moderate benefit but remained bedridden. Two patients gained no relief. Elevated bilirubin and inability to tolerate rapid increase in vascular volume and excrete the fluid load contraindicate the procedure, but the LeVeen shunt appears to be of significant clinical benefit in selected cases of malignant ascites.

AbstractLymphocyte activity of patients with gastric cancer and its relation to the stage of cancer were studied. PHA response of lymphocytes from patients with gastric cancer was decreased markedly even in early stage of cancer.
