159 phakic eyes with chronic open-angle glaucoma were treated by argon laser trabeculoplasty and followed for 5 years. According to our criteria of failure, 31 eyes were failures at 1 year (19%), 47 eyes at 2 years (30%), 66 eyes at 3 years (41%), 77 eyes at 4 years (48%) and 82 eyes at 5 years (52%). In the 77 eyes still controlled at 5 years, the intraocular pressure drop was 6.57 mm Hg and the initial medical treatment could be tapered or stopped in only 33 of the cases (43%). In the 82 failures, there was a greater percentage requiring surgery for pressure control during the first year after laser treatment: 16 trabeculectomies in 31 failures (52%). The initial mean intraocular pressures of 7 failures needing surgery during the first 2 months (27.22 mm Hg) and of the 27 failures needing surgery during the 5 year follow-up (24.41 mm Hg) were higher than the initial mean intraocular pressure of the 77 successful cases (22.67 mmHg). In the 159 eyes followed for 5 years, 19% (31 eyes) to 6% (5 eyes) new failure cases per year were encountered and the initial good results obtained by argon laser trabeculoplasty persisted for about 5 years in half of the cases.

To investigate the influence of the intensity of coagulation spots on retinal function and the clinical course after panretinal argon laser photocoagulation in diabetic retinopathy, we conducted a prospective study in 24 eyes of 12 diabetics. One eye was treated with moderate spots (average: 300 mW), the fellow eye was coagulated with intense spots (average: 600 mW). The spot size was identical in both eyes. Subjective parameters (visual acuity, perimetry), as well as objective functions (ERG, EOG) and the clinical course, were studied preoperatively and on a regular base with a follow-up of 12 months. Visual acuity and fundus findings deteriorated less often in eyes coagulated with intense spots. Visual field loss, however, was more prevalent in eyes treated with intense spots. Early treatment complications only occurred with high-energy coagulation. For this reason, high-energy spots should not be used even though they might be indicated theoretically.

The rapidly alternating combination of Nd:YAG laser power of the optical breakdown mode with cw energy of the argon laser ablates tissue. Blood agar provides a standard test medium as tissue substitute and permits to quantify the effect of various laser energy combinations to achieve optimal ablation. High wattage of the Nd:YAG laser provides destructive pressure that creates fissures which act as the light traps of a black body simulator for efficient cw argon laser energy conversion to pressure and kinetic energy. Within the fissure the deposited cw argon laser energy is directed by gradients toward the tissue surface so that deepening troughs or tunnels are formed by expelling tissue debris.

A new method for refractive measurements with atropine cycloplegia was tested in 90 children with squint. Measurements were taken by an autorefractor (Canon R10) 90 min after application of 2 atropine eye drops and after 3 days of receiving 1 atropin eye drop 3 times daily, i.e. 9 drops per eye. Results were also compared to conventional “objective” refractometry by means of a hand refractometer (Rodenstock PR 50). According to this method, the total refraction, spherical and cylindric power and the cylinder axis are in agreement with refractive data in 80–90% of the children after 3-day atropinization. The additional cycloplegic effect of atropine given for 3 days is only 0.5 dptr compared to the application of 2 drops on the first day. So for practical purposes the refractive data evaluated after 90-min atropinization can be used as a basis for the prescription of glasses.

In order to obtain a new diagnostic tool for the early diagnosis of glaucoma, we developed a method based on computerized analysis of the optic disk, cup and neuroretinal rim areas. A fundus camera, a personal computer and a graphic tablet are employed. Suitable software was developed for calculating the areas (disk, cup and rim) providing an index, the rim/disk ratio. The method revealed high repeatability and reproducibility. The simplicity and speed of the procedure make this test suitable for routine clinical use in the early diagnosis of glaucoma.

In a prospective randomised study we tested two non-steroidal anti-inflammatory medications clinically in a group of 40 eyes. 20 eyes received topically administered indometacin and another 20 flurbiprofen eyedrops. Before we started the planned extracapsular cataract extraction we performed a puncture of the anterior chamber and aspirated a small volume of anterior chamber fluid. Indometacin is well known as local eye drug for the prevention of cystoid macular oedema. The concentration is 50mg/5ml. Another inhibitor of the prostaglandin synthesis is flurbiprofen; its concentration is 1.5mg/5ml. Indometacin is a suspension and available in different concentrations dependent on the shaking procedure. We measured the concentration of indometacin in one single eyedrop from a full bottle, half full and a nearly empty bottle. After correct shaking the concentration of indometacin was between 48 and 53mg/5ml.

The perceptual latency (PL) to detect stimuli during visual field measurement is not a usual routine test. An automated perimeter (‘moniteur ophtalmologique’) was used to measure the central 30° of vision with static targets in 50 normal subjects. The visual field was divided into four areas. The purpose of this study was to compare the PL in each area. An increase in the PL was observed, the nasal superior area being longer than the inferior temporal area. It seems that this psychophysical test is transmitted by a specific class of retinal ganglion cells: Y cells. We believe that PL could be a tool for detecting retinal ganglion cell damage.
