The most frequent changes of the anterior eye observed in senescence include changes in refraction which may be toward the hyperopic or the myopic side and a diminished tear secretion which may lead to the sicca syndrome. The corneal endothelium often shows degenerative changes which may lead to a decompensation of the cornea and corneal edema. A senile cataract is a frequent finding in older people as is chronic open angle glaucoma. One of the most serious senile changes is macular degeneration which may lead to extremely poor central vision though the peripheral vision may still be satisfactory. A similar effect has the less frequently seen anterior ischemic optic neuropathy.

Congenital epiphora due to absence or imperforation of lacrimal puncta and/or canaliculi was present in 21 members from five consecutive generations of a Sicilian family. The anomaly was transmitted as an autosomal-dominant characteristic with apparently complete penetrance and highly variable expressivity.

This double-blind study suggests that oxatomide may be considered a first-line alternative in the treatment of allergic follicular conjunctivitis. For 4 weeks 18 patients were treated with oxatomide at a dosage of 30 mg b.i.d. A randomized control group of 17 patients received a placebo. Already after 2 weeks, and more marked after 4 weeks, oxatomide produced a significantly higher reduction of itching and follicles than the placebo, involving a lower use of supplementary antihistamine medication. Globally, an excellent or good result was obtained in 78% of the oxatomide-treated patients but only in 29% of the placebo-treated patients. In the drug group 1 patient had gastrointestinal complaints and 1 felt slightly sleepy.

Aphakic retinal detachment must nowadays be considered as a specific entity among retinal detachments. In the light of statistics, phakcoextraction cannot any longer be conceived as a simple operation, and the aphakic patient as a normal subject. Aphakia might even be classified as a real disease (aphakosis), as a number of vitreoretinal disorders seem to be specific of this condition. The authors studied 600 aphakic eyes, of which 300 were prophylactically treated by circumferentia, equatorial argon laser photocoagulation. Of the 300 eyes subjected to argon laser photocoagulation, only 1 exhibited retinal detachment (0.33%) versus 6 (2%) in the other group. These results are similar to those obtained by others with different preventive techniques ; but the undeniable advantages of argon laser photocoagulation makes the most suitable method for the prophylactic treatment of aphakic retinal detachment.

In the right eye of a 2½-year-old boy a markedly prominent tumor, arising from the posterior pole, was found. The clinical diagnosis was retinoblastoma and the eye was removed. Histologically, however, this appeared to be a medulloepithelioma originating in the optic disc and in the posterior retina: an extremely rare localization as these tumors usually originate in the ciliary region. The tumor mainly consisted of neuroectodermal elements in several degrees of differentation giving the aspect of a benign (hamartomatous) embryonal brain tumor rather than those of a retinal tumor. Some aspects of the nomenclature are discussed.

A 21-year-old male patient with non-Hodgkin lymphoma of the ‘convoluted’ T cell type and marked involvement of skin and central nervous system presented in the early stages of the disease with subconjunctival bleeding of the eyeballs and massive serous retinal detachment near the optic disc in both eyes. In addition, there was a severe disseminated intravascular coagulation (DIC) caused by the lymphoma. It appears that the non-haemorrhagic retinal detachment on both sides is due to a neoplastic lymphocellular infiltration with high affinity of this malignant lymphoma to the central nervous system. Treatment of the underlying disease and of DIC with rapid onset of total remission lead to resorption of the conjunctival haemorrhages and fast reduction of the retinal detachment.

The EOG was determined in 130 normal subjects. The mean value was 2.42, the interindividual standard deviation 0.42. The difference between the right eye and the left eye was not significant, neither was the difference between a group of youngsters (20–30 years) and an older age group (50–55 years). In another group of 48 normal subjects, 10 or more EOGs were determined per individual. The intraindividual standard deviation was not a fixed number, but appeared to be dependent on the height of the EOG, being approximately 10% of the EOG value. This is a confirmation of one of our previous observations.

A woman, aged 57, reported a considerable loss of vision in her right eye within approx. 60 s after lying down. She regained vision in the upright position within approx. 90 s. The Ganzfeld ERG, recorded in the upright position, showed a moderately reduced amplitude, but was entirely absent when recorded in the supine position, closely correlated to the subjective fading of the vision. Although all other clinical tests revealed no further hint to the cause of this remarkable amaurosis fugax, a temporary hypoxy of the retina due to circulatory deficiencies is suspected.
