Beginning in May 1984, we performed penetrating keratoplasties in combination with extracapsular cataract extractions and implantations of posterior chamber lenses in 21 patients. In addition to cataracts, 15 of these elderly patients presented Fuchs’ dystrophy and 6 had corneal scars due to infectious or inflammatory corneal diseases. In 2 of the 21 eyes, keratoplasty was a repeat operation because of graft rejection or graft dystrophy. With a mean follow-up of 22 months, visual rehabilitation was satisfactory: 15 patients (71%) achieved a postoperative corrected visual acuity of 6/12 or better. Postoperative deviation from preoperatively calculated refractive lens power – the only major problem – remained within ± 2 dptr in 95% (20 eyes), with an error of +4 dptr in 1 eye (5%). In all 21 eyes, clear corneal grafts were obtained. Thus we consider the triple procedure to be the treatment of choice in cataract patients with corneas opacities.

We report on a patient in whom a cytomegalovirus infection occurred in connection with AIDS. Together with the alterations of the fundus typical for cytomegalovirus retinitis, reticular deposits were observed on the corneal endothelium. Specular microscopy showed occasional endothelial cells with edema, as well as small, dark, round, mobile structures (probably lymphocytes) and large, bright, spindle-shaped, immobile structures (probably macrophages) located behind the endothelium. Despite treatment with corticosteroids and acyclovir, an increase in the extent of the findings was observed. The deposits are interpreted as a reaction of the endothelium to the cytomegalovirus infection.

Twenty-two eyes of twenty-two patients suffering from retinal peripheral degenerations underwent preventive equatorial buckling with argon laser photocoagulation. The fellow eye was used as control. We evaluated the effects of laser photocoagulation on corneal sensitivity, on accommodation, on pupillary diameter and light reflex. In the postoperative period, up to the 3rd month, results were significant with respect to the control eye. The mechanisms to which the onset of these undesired alterations is due are discussed.

A 22-year-old man with mild cicatricial retinopathy of prematurity without retrolental mass developed angle-closure glaucoma following pupillary block mechanism. A large lens and a shallow anterior chamber of the premature eye contributed to this mechanism.

Blood pressure during eye surgery under local anesthesia was measured in 186 cases treated in our department. 58 cases showed an increase in blood pressure before the operation in spite of premedication; in 32 of these cases there was a 40% rise in blood pressure. Moreover, 38 cases showed at least one ECG abnormality just before the operation in spite of no past history of heart disorders. To control the blood pressure in these cases, intravenous injection of nitroglycerin (Millisrol) was performed continuously and immediately stabilized the blood pressure.

When glaucoma medication fails to adequately control intraocular pressure (IOP), a second medication is frequently added. Before adding a second drug to patients whose IOP was no longer controlled by 0.5% timolol, we tested the effect of switching to another beta blocker, levobunolol (0.5 or 1%). We also evaluated the effect of study participation on compliance in the control group continuing to receive 0.5% timolol. In each treatment group, the IOP of approximately 30–40% of the patients was successfully controlled for the 3-month study period. The remaining patients did not exhibit significant pressure reductions and were dropped from the study within 2 weeks. We concluded that (1) the results of’switch’ studies without a control group must be interpreted carefully, and (2) the initiation of a ‘new regimen’ with an equieffective beta blocker may be sufficient to increase compliance and thereby control IOP.

Various ocular abnormalities have been reported in patients with thalassemia, and have been, for the most part, attributed to the toxic effects of high-dose desferrioxamine (DFO). We have performed an electroretinographic and visual-evoked potential (VEP) study in 31 thalassemic patients who had never received high doses of DFO. The abnormalities found were very similar to those reported in early siderosis bulbi and included a b1-wave of significantly higher amplitude at 1 min and at the alpha point. VEPs showed a N1-P1 amplitude significantly greater than in controls. These findings, which were more marked in older patients, point to an important causative role of iron in their genesis.

Fourteen cases of spinocerebellar degeneration were examined. While the patients attempted to keep their eyes in the primary position, horizontal eye movements were recorded for 1 min in total darkness by infrared reflection oculography. Instability of eye position in the dark was observed in 10 patients. The range of the eye position error was 3–6° in 6 patients, 6–10° in 3 patients and over 10° in 1 patient. These results indicate that the cerebellum plays an important role in extraretinal (nonvisual) eye position control.

Grating contrast sensitivity was measured using different colors and different grating orientations. The contrast sensitivity was highest when measured with green light, a little less when measured with red light, and lowest with blue light. The contrast sensitivity with blue light was relatively low compared with green and red light for high spatial frequencies, and relatively high for low spatial sensitivities. The so-called ‘Oblique effect’ was confirmed in this study, but a difference between horizontal and oblique gratings was only found in spatial frequencies higher than 18 cycles/degree. The present study, carried out on 20 persons, suggests that the inter- and intraindividual components of variance are smallest when measured with green light and vertically oriented gratings.
