摘要:
The disorder of developmental dyslexia is reviewed. Clinical manifestations, symptom patterns, and classification systems are outlined. Neuropsychological dimensions of dyslexia, including maturational lag, impaired cerebral dominance, EEG, evoked potential, biochemical correlates, and neuroanatomic abnormalities are discussed with reference to theoretical and research findings.

Eighteen healthy 8− to 11-year-old children with normal hearing and histories of recurrent middle ear effusion (MEE) before the age of 5 years were evaluated by a battery of audiologic, psychologic, language, and achievement tests. Each of these children was compared to a non-MEE sibling who was tested at the same age. None of the children had been diagnosed as learning disabled. Both groups tested in the bright normal range of verbal ability on the WISC-R. Paired comparison of sibling data revealed that the MEE subjects had deficits in verbal ability, auditory decoding, and spelling skills compared with control siblings. Significant strength in visual sequential memory in the MEE subjects suggests the availability of compensatory strategies for auditory deficits in these subjects. Sex and/or birth order differences did not explain the differences observed between siblings.

The response characteristics of premature infants at 8 weekly postconception age levels (33 to 40 weeks) were studied in a cross-sectional design. The consistency of behavioral and reflexive responses was assessed on the Brazelton Neonatal Behavioral Assessment Scale. Age-related (nonsignificant) trends were seen in behavioral responses such as orientation, alertness, motor maturity, and state control. On 12 elicited responses and 14 behavioral items, significant differences were obtained between 33− and 40-week exams. These results suggest that while the maturational processes of a preterm infant may be detected using this scale, it is not, without qualification, an appropriate clinical instrument for use with this group. However, the Brazelton scale may be useful in research on group differences in the developing capabilities of preterm infants.

摘要:
The effectiveness of a home-based early intervention program for children of adolescent mothers was examined in comparison to a control group who received no such intervention. Intellectual assessment at 2 1/2 years of age suggested that the early intervention program can promote developmental functioning and lessen the risk of cognitive delays. Behavioral analysis suggested that the patterns of adolescent mothers' interaction with their children varied as a function of both involvement in the intervention program and the child's risk status, as defined by IQ scores.

The relationship between biological, psychosocial, and demographic perinatal factors and the adequacy of the children's home environment was evaluated in a sample of 69 families 3 years their children had been discharged from a newborn intensive care unit. Almost 40% of the variance (p = .0001) in the amount of home stimulation was explained by income, race, and maternal age. Three maternal perinatal psychosocial characteristics explained an additional 12% of the variance (p = .0001). Both maternal demographic and psychosocial factors appear to contribute to the preschool home environment. Research focused on the precursors of an adequate early home environment could suggest specific interventions for the primary prevention of parenting problems.

Reports are presented of two children with partial complex seizures (PCS) who had a variety of behavioral and psychiatric symptoms that were related to toxic serum levels of anticonvulsants. Anticonvulsant toxicity should be considered in the differential diagnosis of psychiatric symptoms in all children with convulsive disorders. The traditional view that the psychomotor spells have behavioral and psychiatric components increases the complexity of the differential diagnosis. PCS in children present differences in symptomatology and more difficulties in diagnosis and treatment compared with adults.

摘要:
A questionnaire was sent to 83 pediatricians to determine which issues they considered important in the pediatric prenatal visit. Of the 64 respondents, 58 do prenatal interviews. Apporximately half of the mothers of newborns entering these practices had prenatal visits. Most respondents indicated that the prenatal visit focused on meeting the family and discussing practical aspects of infant care. The finding that few respondents focused the interview on psychological aspects of parenthood raises questions about the role of the prenatal visit in pediatric care.

The effects of pre- and/or postnatal infant car safety education were examined. Educational approaches included demonstrations and talks on automobile crash statistics in the prenatal course; and a car safety film on the hospital television, a pamphlet given to each mother, and instructions to nurses to encourage parents' purchase and use of car restraints in the postpartum period. Although differences were not statistically significant, the reported and actual use of infant restraints on the trip home was highest in the pre- plus postnatal education group. Conseling in any period was associated with higher restraint use than no counseling. The pediatrician's role in effecting positive behavioral changes in this and other areas is emphasized.
