摘要:
ABSTRACTThis article discusses the previous and current uses of silicone liners with the Silicone-Only Suspension (SOS) technique. This technique has been enhanced by the use of injection-molded silicone liners, Socket-Loc (patent pending), and the Ring. Previously reported problems with SOS included unwanted internal socket rotation and intermittent loss of suction suspension. Socket-Loc and the Ring have eliminated these difficulties while retaining the many advantages of the SOS technique.

摘要:
ABSTRACTA retrospective study was performed to determine how long a thermoplastic ankle-foot orthosis (AFO) will fit appropriately and when subsequent adjustment and replacement are indicated. Clinical records of cerebral palsy and spina bifida patients ages birth to 21 years who used six types of thermoplastic AFOs from 1982 to 1992 were reviewed. Neuromusculoskeletal functional status appears to determine the AFO design prescribed and its replacement. Adjustments to thermoplastic AFOs primarily involve heat flaring to alter plastic contours and provide sufficient space for donning, doffing and pressure relief over regions of bony prominence. Further research is necessary to confirm the results obtained in this study.

ABSTRACTAlthough anecdotal evidence supports the application of a lateral heel wedge (LHW) as a nonoperative treatment for varum gonarthrosis, objective evaluation of its mechanism of action is limited. Roentgenograms, ground reaction forces and electromyographical (EMG) profiles were incorporated to study the effect of LHW on the medial osteoarthritic knee of seven males during static and dynamic conditions.Results indicated both static and dynamic changes associated with the application of a LHW. Roentgenograms indicated a varum to valgum directional change (p<.05) in the capito-midcondylar-tibial shaft angle during static standing. There was minimal impact on the mean vertical forces while there were increases in the maximum lateral forces (p<.05) during walking with the LHW treatment. No statistical differences were observed for the EMG profiles between conditions. These results provide some insight into the functional effects of the LHW on the kinetics of the lower leg during static standing and free-speed gait.

摘要:
ABSTRACTA quantitative method for comparing external foot shapes has been developed and evaluated for potential application to custom shoes/orthoses and orthopedic research. The system employs a laser scanner and custom software to obtain more than 16,000 points (every 3 degrees radially, every 2 mm proximal-distal) over the entire foot (dorsal and plantar aspects). The high-resolution images are transferred to a computer workstation where the foot shapes are aligned, compared and relative differences between the shapes are graphically displayed.The accuracy, reproducibility and performance of the foot comparison system are well within the tolerances required for clinical use. When comparing measurements on objects of known size and shape to those determined by the system, the maximum aggregate system error (both hardware and software) was less than 1 mm. The numerical algorithm for digital alignment of scanned feet is reproducible to 1 degree and .01 mm translation. The average computer time for comparison of scanned images in this study was 10 seconds; the entire process, including digitizing, took less than 15 minutes.Although this system unrealistically treats the foot and shoe as rigid entities, it is consistent with the goal of providing shoes and insoles that reduce the risk of ulceration in insensate feet during shoe break-in or normal activity.

ABSTRACTA conference conducted Aug. 29–31, 1994, was organized by the National Center for Medical Rehabilitation Research (National Institute of Child Health and Human Development, National Institutes of Health) and cosponsored by the Agency for Health Care Policy and Research to formulate recommendations for needed outcomes research in medical rehabilitation. Recommendations are presented in the topical areas of philosophic issues, strategy and design issues, measurement of disability and handicap, and measurement of quality of life and health status.
