摘要:
More than 6,600 one-page surveys were sent to individuals throughout the country with upper-limb loss or absence. Of those surveys, 2,477 were returned, and demographic information was recorded. A more comprehensive seven-page survey was then sent to the respondents who agreed to participate. A total of 1,575 of these surveys were returned: 1,020 by body-powered users, 438 by electric users and 117 by bilateral users of prostheses.The results of the surveys indicate users of body-powered and electric prostheses identify surprisingly similar elements as necessary in the design of a better upper-limb prosthesis. These qualities include additional wrist movement, better control mechanisms that require less visual attention and the ability to make coordinated motions of two joints. Desired near-term improvements for body-powered prostheses include better cables and harness comfort, whereas those for electric prostheses include better gloving material, better batteries and charging units, and improved reliability for the hand and its electrodes. This article discusses the specific functions that various levels of upper-extremity amputees gain from their prostheses as well as the device features that aid or detract from their functions.

摘要:
The purpose of this study was to evaluate the efficacy of a custom-molded leather ankle lacer for treating painful problems of the ankle and hindfoot. The evaluation involved patient self-assessment, clinical examination and radiographic determination of the effectiveness of the ankle lacer.Overall, patients who used the lacer experienced moderate pain relief with significant but not complete restriction of motion. Based on this study and clinical experience, the authors concluded the leather ankle is a compliant and comfortable treatment strategy for patients who desire some retained motion of the ankle and hindfoot without intolerable pain.

摘要:
The malpractice litigation crisis affects all clinical health-care professionals. Providers are sued by patients for professional (treatment-related) negligence, injury from defective products, breach of contract, and/or intentional acts and omissions. Because orthotics and prosthetics (O&P) focuses on both professional product and service delivery, prosthetists and orthotists are particularly vulnerable to professional negligence and product liability claims. While it is vital to maintain an altruistic focus in care delivery, clinicians and managers also must practice effective malpractice risk management. Of particular importance is the generation of legally acceptable incident reports when patients are injured during evaluation or treatment.

摘要:
The purpose of this article is to share the authors' experiences fitting a very young figure skater who has bilateral transtibial amputations with different components during her first seasons of skating as an amputee. Her first ice-skating prostheses incorporated modified articulated feet for children, allowing plantarflexion with some additional dorsiflexion at the ankle. However, more movements and push-off were needed to accommodate jumping and more difficult ice figures.A Dual-Ankle Spring, Multi-Axial Rotation System (D.A.S.-M.A.R.S.) unit with a Seattle Light Foot was then fitted on each prosthesis to provide torsion motion as well as evasion, inversion, plantar- and dorsiflexion. As a result, the subject's ice-skating abilities improved, and more precise movements, jumps and turns were accomplished. Subjectively, these improvements were greatly appreciated by the young skater.

摘要:
The purpose of this article is to present the concepts involved in reader evaluation of research literature. Knowledge gained through research is valuable only if it is shared with others. It is the duty of the members of a profession to critically review their professions literature, challenge claims that seem unreasonable and champion those that elevate the profession. Better patient care and increased professional growth will result when clinicians learn to evaluate and make use of the research literature.The essential elements of a research article are the title, abstract, introduction, method, results, discussion and conclusion. The introduction should contain the following elements: statement of the problem, literature review, purpose and expected results (hypothesis). The method should define the subjects, instrumentation and apparatus, procedure, and data analysis. The data analysis is divided into statistical tests for continuous data and discrete data. The results section should succinctly present the results with no interpretation of their meaning. The discussion is where the knowledge and insight of the author(s) are allowed to bloom.This article identifies the polarization between clinician and researcher, and readers are enjoined to help educate both callings of the O&P profession. The profession will be elevated with the development of research-oriented clinicians and clinically oriented researchers.
