摘要:
A study was conducted to determine the benefits of prosthetic training for a person using an upper-extremity prosthesis. Ten individuals without amputations were randomly assigned to either a training or nontraining/control group. Since these individuals were not amputees, they were fitted with bilateral training prostheses.The training group was involved in a training program administered by the researcher in which the participants took advantage of various tools (blocks, paper, pencils, clothes and hangers) to learn how to use upper-extremity prostheses. The control group received the same tools and performed the same activities as the training group, but members were not trained in the same manner as those in the first group.Subjects were involved in four sessions of approximately two hours each. Pre- and post-tests determined the progress of the subjects. In addition, all subjects were required to complete four additional tasks to determine if the groups would apply what was practiced in the sessions.The University of New Brunswick Test for Prosthetic Function was adapted for assessment of prosthesis use. Statistical analysis was accomplished through the use of the Mann-Whitney U Test (Wilcoxon Rank Sum Test). The individuals who received training performed tasks in a skillful, efficient manner, exceeding the performances of the untrained group.

摘要:
Over the past several decades, the evolution of gait science has produced an array of terms and concepts relating to observational gait analysis. Prosthetists and orthotists use various forms of gait analysis on a daily basis as an important part of clinical care.When the basic principles of normal walking are understood, a more penetrating grasp of pathological gait becomes possible. The result is expanded ability to differentiate between pathological and compensatory gait deficits. In addition, efficient interaction with the clinic team demands a sound conceptual knowledge base of human locomotion and related terminology. This will facilitate an optimal treatment plan for the patient and enhance communication and prescription recommendations to the physician and relevant paying agencies. This article is intended to be an introduction to gait science with these goals and objectives in mind.

摘要:
The OTTO BOCK 3R60 prosthetic knee joint allows controlled flexion of the prosthetic knee under weightbearing in early stance phase, more closely simulating this aspect of gait than was previously possible. As the amputee transfers weight onto the prosthesis in early stance phase, this knee gradually flexes up to 15 degrees, thereby cushioning the impact of weight acceptance.In contrast to all previous polycentric knees, stability of this new design is biomechanically increased by stance flexion, rendering a locking feature unnecessary. Clinically, the amputee perceives this prosthesis as stable and comfortable. The biomechanical features of this design concept, initial clinical observations and preliminary conclusions from laboratory gait analysis are summarized.

摘要:
The authors conducted a study of 45 patients with anterior cruciate ligament- (ACL-) deficient knees who had been fitted with rigid knee orthoses in the past three months. In addition to, being diagnosed with knee pathologies, all patients had complained of pain and “giving way” (instability during physical activities).The study was undertaken for two reasons: 1) to gain insight into the usability of two custom-made and two “off-the-shelf rigid knee orthoses based on their actual use, usefulness and ease of use; and 2) to determine the influence of the type of orthosis on patient outcomes.Almost all of the orthoses actually were used at home and in leisure time as well as at work and in sports. Usefulness of all types of rigid knee orthoses was the greatest in circumstances when the patients performed poorly before orthosis fitting. Ease of use played a secondary role. The four types of rigid knee orthoses did not significantly differ in respect to the research objectives.

摘要:
Heterotopic ossification (HO) is a common complication following spinal cord injuries, traumatic brain injuries and burm. Although bony overgrowth has been shown to occur after acquired pediatric amputation, there are no reports linking HO to amputation in the adult population.The authors review the literature and present a case report involving the unusual finding of heterotopic ossification following traumatic transtibial amputation in an adult patient. A 50-year-old Bosnian man sustained multiple shrapnel wounds to the lower extremities due to artillery shelling at the “Bread Line Massacre” in Sarajevo during May 1992, which resulted in bilateral transtibial amputations. One year later, with the assistance of the International Organization for Migration, he was brought to the department of physical medicine and rehabilitation at the Medical University of South Carolina for prosthetic fitting and gait training.Roentgenograms revealed both residual limbs had developed heterotopic ossification that extended up to 4 cm beyond the tibia and fibula of the distal residual limbs into the subcutameous tissue. The patient was fitted with bilateral patellar tendon-bearing prostheses with Springliteadynamic response feet. Despite the presence of the extensive ossification, the patient became a successful prosthesis user and achieved independence in all activities.

摘要:
This article discusses the postoperative rehabilitation success of a 42-year-old white male patient who underwent discectomy cnd fusion of the L5-S1 vertebrae in 1989. The operative procedures were a technical success, but the patient continued to suffer from chronic low-back pain symptoms and remained unemployed and on disability. He was particularly sensitive to forward flexed postures and exhibited low tolerance for exercise.The patient was fitted with a Bending Non-Demand Return (BNDR®) lumbosacral orthosis (LSO) designed speclfically for dynamic assistance white in the forward flexed posture. Results were dramatic; back range of motion and mobility were substantially improved with increased tolerance for exercise, resulting in a weight loss of 34 lbs and improved quality of life. The authors believe this to be the first application of a dynamic orthosis to aid in low-back rehabilitation.

Practitioners must have the ability to investigate and add to the orthotic and prosthetic body of knowledge, Likewise, the ability to critically review research is essential.Recognizing this, the University of Texas Southwestern Medical Center at Dallas Prosthetics-Orthotics Program has, over the past eight years, designed its curriculum to culminate in a senior research project demonstrating the students' mastery of the essendal tools required to perform research. It begins with coursework in statistics and computer skills prior to acceptance. Early in the prosthetics-orthotics curriculum, students are introduced to available campus resources. Students gain experience in research methods through a series of seminars and a scientific inquiry course. During their senior year, students conduct research and present the findings in written, oral and poster forms. Several senior projects have been published in professional journals. Established guidelines assist students and their faculty research advisers in successfully carrying out the projects.Any stttdent or faculty research endeavors at the university are monitored by the Institutional Review Board, Office of Grants Management and the program itself. Student researchers also may contribute to ongoing faculty research and conduct pilot studies. Graduates trained in research methods have the potential to make meaningful contribution to the field.
