A review of recent literature indicates several divergent perspectives are used currently in managing patients with spastic lower extremities. The intent of this article is to analyze methods presented in published clinical orthotic studies regarding biomechanical and neurophysiological approaches to tonereducing orthoses. Each management rationale will be briefly summarized, including biomechanical, primitive reflex activity, pressure over muscle tendons, prolonged stretch, and orthokinetics. Results of lower-extremity orthotic studies will be reviewed. Discussion will focus on current research efforts and indications for future clinical practice.

The use of spinal orthoses for chronic low back pain has traditionally been random and empirical. Frequently, a lumbosacral corset is prescribed and, upon failure of the corset to reduce pain, orthotic treatment is abandoned. In this prospective study, the test instrument of Willner, consisting of a rigid aluminum thoracic lumbosacral orthosis (TLSO) with a sagittal lumbar pad that is adjustable in lumbar flexion or extension, is used to test the hypothesis that a five-day trial in a test orthosis will be a good predictor of the outcome of orthotic treatment for chronic low back pain.

This article will discuss an early postoperative patient management technique, below-knee polyethylene semi-rigid dressings. This approach is a contemporary alternative to the directly molded removable rigid dressing. The polyethylene socket dressing is lightweight, washable and flexible to enhance patient comfort and acceptance.

Ninety-four patients from two amputee clinics completed a 123-item questionnaire. Results were tabulated and subjected to statistical analysis by computer. Results indicate patients feel decreased ability to defend themselves, a sense of difficulty with appearances, and perceive they did not receive rehabilitation care in transfers, tub baths, driving, etc. Tests for depression were more strongly positive in patients who most recently had undergone amputation.

Four experimental axillary crutches, along with conventional crutches, were tested by five disabled subjects and five normal subjects, with knees flexible and with knees immobilized to simulate paraplegic conditions. Experimental crutches included a suspension crutch, rocker crutch, spring crutch and prosthetic foot crutch. Active and resting heart rates were recorded, along with average velocity to calculate an energy expenditure index for each trial. No statistically significant energy savings were observed with the experimental designs. However, the suspension crutch was the most energy-efficient of the experimental crutches and is worth exploring further. In addition, immobilizing the knees of normal subjects is judged to be an effective method of simulating paraplegic crutch ambulation.

In the past, people who managed to survive the traumatic loss of a leg or even those born with a major limb deficit required an ambulatory aid for functional mobility. The peg leg was developed under these circumstances and has remained with us throughout history. Since it continues to be used by many amputees regardless of other options they may have, the peg leg remains valuable. Prosthetics Research Study has designed a new lower-limb terminal device called the All-Terrain Foot.
