ABSTRACT:This is a report of the differences and similarities between two groups of 30 women each who selected widely divergent types of childbirth care. One group chose to deliver their babies in a tertiary university hospital obstetric service, the other in an out‐of‐hospital alternative birth center staffed by nurse‐midwives.Women choosing the birth center were not demographically different from those choosing the tertiary hospital except that they were somewhat older. However, women choosing the birth center planned to emphasize autonomy and independence rather than intimacy in their child rearing, and they described their partners as much more supportive and involved in the birth, and were much more adaptive in preparation for the birth and the baby's

ABSTRACT:Twenty‐three women who sought out prenatal (22) and parenting (23) classes scored in the normal range for physical complaints, ego strength, depression, anxiety, and the lie scale of the Minnesota Multiphasic Personality Inventory (MMPI) at an average of 3.5 months postpartum, and had improved scores overall when tested again six weeks later. ABSTRACT:A review of the medical literature shows that episiotomy is done in about two‐thirds of births in the US, and routinely in many settings, especially in primiparas. While episiotomy can be clinically justified in specific circumstances such as impending laceration of the perineum, the literature does not support its routine use, since it is associated with significant risks of pain and infection, and the benefits claimed for routine episiotomy have not been subjected to randomized controlled trial

ABSTRACT:Childbirth education for parents who elect to attempt a vaginal birth after cesarean (VBAC) has many of the same elements of traditional childbirth education, including the imparting of knowledge and confidence, training in breathing and relaxation techniques, and helping parents to arrange for the most supportive environment and caregivers. Special differences in VBAC parents include the need to mourn the previous loss of an ideal birth, to analyze why they want a VBAC, and to understand the reason for the previous cesarean and realistically assess the chance of recurrence. The fear of uterine rupture or scar separation afflicts both parents and physicians, but has been shown to occur no more frequently than in repeat cesarean. By one analysis the elective repeat cesarean is a higher risk procedure than is a trial of labor.

ABSTRACT:A childbirth educator describes burn‐out as having been due to boredom, frustration and exhaustion. She overcame burn‐out by changing focus in her classes, and by introducing skills learned in courses in teaching the adult learner, group dynamics, and assertiveness training. Many new books were added to her lending library, and she joined two new organizations in the childbirth field. She drew up a Prepared Childbirth Preferences form to help couples talk to their doctors during pregnancy. ABSTRACT:The New York trial of prenatal protein supplements by Rush et al., together with the study by Stein et al. of the Dutch Hunger Winter, are reviewed to show the importance of maternal nutrition around the time of conception. Both studies reinforce the view that embryonic hypoplasia and dysplasia early in pregnancy cannot be reversed by good nutrition, or protein supplements, during the last half of pregnancy. The high perinatal casualties in both studies were among populations of women with body weights around the Harvard infertility threshold at the time of conception. The population studied in New York was not short of protein and the supplements caused nutritional imbalance for some patients
