A retrospective review of 37 cases of infantile tibia vara (25 patients) demonstrated a high rate of recurrence following proximal tibial and fibular osteotomy. Sixteen limbs (43%) required a single osteotomy, whereas 21 (57%) required from one to four additional osteotomies. We conclude that in patients with infantile tibia vara, massive obesity, initial osteotomy with Langenskiöld grade III or greater lesions, and initial osteotomy after 4 1/2 years of age are all associated with a high degree of recurrent deformity.

Six children (nine limbs) underwent fibular centralization for complete paraxial tibial hemimelia. The preoperative, intraoperative, and postoperative criteria as described by Brown (J Bone Joint Surg |Am|1965;47:695–704) were strictly met. At initial postoperative evaluation, three limbs had active knee extension, and live limbs had minimal flexion contractures. However, at final follow-up, 20–123 months later, all knees had ligamentous instability and poor active range of motion: eight had significant flexion contractures. All were classified as poor results. Based on this series and other reports, it is recommended that knee disarticulation instead of fibular centralization be performed for complete paraxial tibial hemimelia, preferably within the first year of life.

Three cases of congenital pseudarthrosis of the fibula are reported, all with prepseudarthrotic involvement of the omolateral tibia. In patients with this condition, correction of valgus deformity should be deferred until puberty, and the occurrence of tibial fractures should be prevented by means of leg splints.

The purpose of this article is to describe the technique of performing the Lambrinudi triple arthrodesis. The only change from the way the technique was first described by Lambrinudi in 1927 is the use of staples to hold the correction, once the appropriate sections of bone have been removed. Thirty-three feet in 28 patients have been reviewed. The etiology of their problems, their previous surgeries, and the outcome were reviewed, with an average follow-up of 3.6 years after the operation. All operations were done for pain, deformity, or both. The operative steps are illustrated, and three complications are described.

The surgical treatment of bifid great toes has not generated as much interest as that of its upper extremity counterpart. Early treatment of this foot deformity is necessary for ease of shoe wear and cosmesis. Five feet surgically corrected by a variation of the Bilhaut-Cloquet procedure were followed up for 4 years. Because of postoperative nail bed deformities, the nail splitting incision of Bilhaut-Cloquet should be avoided.

Thirty-six extraarticular subtalar arthrodeses were reviewed retrospectively to ascertain our long-term results and to determine the better grafting material, tibial corticocancellous bone or a fibular strut graft. The 21 patients had flexible planovalgus deformities due to various etiologies. Overall, satisfactory results (fusion, no pain, and correction of the deformity) were achieved in 83.3% of patients and unsatisfactory results (residual pronation) in 8.3%, and failures (marked pronation or graft resorption) occurred in 8.3%. The fibular strut graft had a better overall success rate, fewer complications, an easier acquisition, and achieved union where the tibial corticocancellous graft failed.

Calcaneal apophysitis (Sever disease) is a common cause of heel pain, particularly in the athletically active child. Eighty-five children (137 heels) with calcaneal apophysitis were reviewed. Both heels were affected in 52 (61%) patients. The most common associated foot condition was pronation, occurring in 16 patients. Sixty-eight patients complained that pain was made worse by a specific sport, with soccer leading the list. All patients were treated with a physical therapy program of lower extremity stretching, especially of the heel cords, and ankle dorsiflexion strengthening. Soft Plastizote orthotics or heel cups were used in 98% of patients. Proper athletic shoewear was advised. All patients improved and were able to return to their sport of choice 2 months after the diagnosis. There were two recurrences.

Eighteen patients (28 feet) with spina bifida and arthrogryposis had talectomy for correction of severe equinovarus deformity. In 26 feet, posteromedial release had been performed previously. The average age at surgery was 4 1/2 years, with a range of 1–9 years of age. The length of follow-up averaged 4 years and ranged from 12 months to 8 years. Twenty-three feet were rated good, and five were poor. Six feet, because of forefoot adduction, required further surgery (metatarsal osteotomy). Talectomy is an effective procedure for correction of hind-foot deformity. Forefoot problems must be treated as a separate entity.

One hundred twenty-four patients (174 feet) were treated for clubfoot deformity, with an average follow-up of 7 years. All patients were initially treated by manipulation and serial casting. Early soft tissue surgeries were performed for resistant cases (73 feet). In 33 feet with residual hindfoot deformity only, posterior release was performed with 85% good results. In the remainder, where there was forefoot as well as hindfoot deformity, full posteromedial release was necessary, giving good results in 90% of cases. All clubfeet should initially be treated conservatively. However, resistant cases should have early surgery, with the extent of surgery performed being determined by the type of residual deformity present.

A simple form of overhead olecranon traction has been used to treat 27 children with severely displaced supracondylar fractures, in whom a satisfactory reduction could not be obtained by closed means. All the children were assessed clinically and radiologically at an average of 54 weeks after injury. Twenty-two children (81%) had an excellent result, with preservation of the normal carrying angle and no significant loss of movement; the other five children had a good result. Only two children (7%) were left with a minimal cubitus varus deformity. The method is technically easy to perform and has not been associated with significant complications.
