Abstract:This is a report on the clinical experience in consultation‐liaison psychiatric service for patients with end‐stage renal disease undergoing renal transplantation. Among a total of 30 patients (23 men and 7 women) 13 (43%) developed psychiatric disturbances: depressive state, 8; anxious‐irritable state, 5; and delirious state, 1 (One patient had both depressive and delirious states). Five patients got depressed following the episodes of a rejection reaction. Three of them had a graftectomy which exacerbated their depressions. The occurrence of rejection reaction was thought to be a major precipitating factor. Four patients became anxious‐irritable within a week following transplantation. Patient's intolerance to stressors seemed more responsible for the anxious‐irritable state than stressorsper se.Steroid medications were thought to be a predisposing, not a causal, factor to psychiatric disturbances in transplant patients. In the 21 patients who were assessed preoperatively and who had no rejection reactions, the Basic Rorschach Score of less than – 20 was a predictor of posrtransplant psychiatric disturbances. Psychiatric interventions were done for the 13 mentally ill patients in collaboration with the surgeons in charge and ward nurses. For the patients with the anxious‐irritable state, listening, reassurance, and anxiolytics (sometimes with additional antipsychotics) were effective. Abstract:Senile dementia is irreversible and runs a relatively chronic course, causing physical emaciation and psychological burden to families who must take care of senile dementia at home. However, there has been no report concerning a psychological study of families with senile dementia. Therefore, the psychological aspects of them were examined. Concerning the results, most caretakers of Alzheimer‐type dementia with markedly associated psychiatric symptoms showed notably neurotic and depressive tendencies. By family parameters, these tendencies were strongly observed in families of caretakers alone. These results suggest that an expansion of local medical care, specifically geared to the home, is required, and more attention needs to be paid to the psychological aspects of caretaker

Abstract:The Tokyo Autistic Behavior Scale (TABS) consisting of 39 items provisionally grouped in four areas–interpersonal‐social relationship, language‐communication, habit‐mannerism and others–is an instrument used by a child's caretaker to rate the child's autistic behaviors on a 3‐point scale. Test‐retest reliability was satisfactory (i.e., an r for a total score was .94). Among six DSM‐III diagnostic groups, infantile autism showed a significantly higher total TABS score than the other five groups, and a taxo‐nomic validity coefficient was .54. An r between total scores of the TABS and the Childhood Autism Rating Scale–Tokyo Version was .59. The area scores showed a lower validity than the total score. Abstract:In order to understand how psychiatric problems are distributed in general medical departments, we used DIS (Diagnostic Interview Schedule). The subjects are 307 inpatients and outpatients in the Departments in Internal Medicine and Orthopedics of Fukuoka University Hospital, M Hospital and N Hospital. As a result, 53.4% of all the subjects showed some psychiatric problems. These are, in a descending order, tobacco dependence (30.0%), psychosexual dysfunction (14.3%), alcohol abuse/dependence (14.0%), major depressive episode (6.5%), organic brain syndrome (4.9%), obsessive‐compulsive disorder (3.9%), dysthymic disorder (2.3%), panic disorder (2.0%) and others. Also, we discussed comparison between internal patients and neurotic patients who visited psychiatrists complaining of physical symptoms, and the incidence of DIS diagnoses in individual physical disease

Abstract:We present here two cases of reversible hypoproteinemia which could have occurred as an adverse effect of chronic lithium administration. In the present cases, protein losing a renal dysfunction, liver dysfunction and malabsorption syndrome were not observed, and the relationship between their dietary volume and serum protein levels was poor. Abstract:We present here a case with various physical and neuropsychiatry symptoms caused by the administration of carbamazepine. The patient suffering from right opthal‐mic neuralgia showed fever, eczema, erythema, lymphoadenopathy, eosinophilia, vomiting, headache, dizziness, nystagmus, and various mental disorders which consisted of emotional instability, personality change, delusions of reference and persecution, depressive state, and hyperventilation syndrome during the administration of carbamazepine. The physical symptoms in the present case were conformable to the side effect of carbamazepine. The mental disorders appeared in a few days from the start of carbamazepine administration and disappeared after the discontinuation of the administration of this drug without antipsychotic therapy and have never relapsed until now. The mental disorders and the physical symptoms were in parallel with their clinical course. This kind of mental disorders induced by carbamazepine has not yet been reporte

Abstract:The English “ward‐in‐a‐house” for new long‐stay patients (NLS) is described. NLS are usually defined as (a) aged between 18 and 65, (b) continuously in hospital for between 1 and 5 years, and (c) individuals for whom it has not been possible to find alternative accommodation outside hospital, despite repeated attempts to do so. Abstract:The effect of L‐threo‐3, 4‐dihydroxyphenylserine (DOPS) on plasma Cortisol, prolactin, thyrotropin‐stimulating hormone (TSH) and growth hormone concentrations was studied in nine healthy male volunteers. The drug was administered orally (300 mg or 600 mg DOPS) using a multiple crossover placebo‐controlled study design. Plasma hormone concentrations were measured at 30 minute intervals for 3 hours after dosing. Plasma DOPS peak concentrations were observed between 2 and 3 hours after dosing. DOPS, however, had no effect on plasma hormone concentrations and this may be attributed to the known low brain permeability of DOPS in healthy

Summary:Inherited deficiency of acetaldehyde dehydrogenase type I (ALDH‐I) was found in 43% (50/117) of normals, 33% (27/82) of schizophrenics, but only 4% (5/113) of alcoholics among Japanese. Abstract:EEGs were recorded for Fpl, C3 and O1 once a week for 4 weeks after the withdrawal of medazepam, a benzodiazepine with a long half‐life, from 7 normal males. Quantified analyses of them showed 2 types of change: A maximum change during the administration of medazepam that later tapered off. It consisted mainly of an increase in fast wave, and was uniform across areas though most significant in C3. A rebound phenomenon that reversed the change produced by the drug, the decrease in fast wave being significant in C3 during week 2. These results suggest that fast wave is the most sensitive EEG element and these 2 types of change must be investigated hereafter in terms of significant changes after the discontinuation of medazepa
