SummaryOne hundred hospital in‐patients treated for pulmonary embolism (PE) and/or deep vein thrombosis (DVT) were randomly allocated to receive 3 or 6 weeks' anticoagulation with heparin and warfarin. At one year recurrence rates were 12% in the 6 week group and 10% in those treated for 3 weeks. No patient died as a result of recurrence. Our study suggests that 3 weeks' anticoagulation therapy, using intravenous heparin for the first 5 days and warfarin from the third day, is adequate for patients without persisting risk factor

SummaryCFU‐GM clonal growth was examined in bone marrow from patients with myelodysplastic syndrome (MDS) in the presence of Ara‐C at 10−8mand 4 × 10−8mand the response compared to the effect on progenitors from normal subjects. Seventy‐three percent of marrow cultures from patients with MDS showed colony growth below the 95% confidence limits for normal subjects in the presence of 10−8mAra‐C. At 4 × 10−8mAra‐C 43% of marrow cultures had subnormal colony growth. These results suggest that CFU‐GM from patients with MDS may have a greater than normal

SummaryAntithrombin III is the major physiological inhibitor of thrombin, and congenital deficiency of antithrombin III is associated with increased risk of venous thrombosis either spontaneously or following trauma, surgery or pregnancy. The successful use of antithrombin III concentrate during and following surgery to prevent venous thrombosis is described in a previously asymptomatic man with familial antithrombin III deficiency.

SummaryError rates in exercises in compatibility testing varied between 3% and 36% over the six years 1979–1984. Evolution of serological procedures was continuous through this period but without clear evidence of improvement in performance of antibody detection although performance in the UK appears to be comparable with that elsewhere. Relationships have been established between a number of variables of reagents and techniques, and their performance. The influence of some variables is reasonably clear but there appear to be interactions between certain variables in their effects on performance, and interpretation is less straightforward. A test for agglutination of enzyme treated cells together with an antiglobulin test appears to be the most reliable combination for detection of incomplete antibodies in compatibility testing although one stage enzyme techniques are an inevitable compromise between reliability and convenience. SummaryThe mode of action of high dose intravenous gammaglobulin (IVGG) in idiopathic thrombocytopenic purpura (ITP) is controversial. This study used immunofluorescence techniques to investigate the possibility that IVGG binds to peripheral blood cells and that such binding blocks attachment of cell specific autoantibodies. IVGG bound to platelets, neutrophils, monocytes and lymphocytes. Erythrocytes appeared to bind only anti‐A and anti‐B present in the IVGG. Multimeric and monomeric fractions of IVGG also bound to these cells. However, F(ab′)2fragments of the IVGG reacted only weakly with platelets, neutrophils, monocytes and lymphocytes. Binding of IVGG to platelets and neutrophils did not prevent opsonization of these cells by monoclonal antibodies with specificities corresponding to those previously ascribed to platelet and neutrophil autoantibodies. SummaryA chemiluminescence technique (CLT) has been developed which measures the interaction between human monocytes and antibody‐coated (opsonized) platelets. This technique has an objective end‐point, is simple to perform and is of comparable sensitivity to the platelet suspension immunofluorescence test (PSIFT) when used to detect anti‐platelet alloantibodies. In contrast, only 4/20 sera from patients with clinically diagnosed autoimmune thrombocytopenia were opsonic in the CLT, while 8/20 of these same sera bound IgG to platelets in the PSIFT. SummaryA microcomputer network is described for the scoring of the differential leucocyte count and the registration of white and red cell characteristics. On each terminal keyboard, keys are assigned to white cell fractions (10), erythroblasts (1), red (13) and white (3) cell morphology and for graduations 1+ to 4+. Duplicate counts on different microscopes are collected by the master, averaged and forwarded to a printer. In the future we hope to transmit results directly to the laboratory computer. The use of the system is not restricted to the differential leucocyte count. An intelligent scoring board can help to alleviate the time‐consuming task of entering qualitative and descriptive results into a laboratory compute

SummaryPreambleScientific symposia on haemoglobinometry were held at the 9th Congress of the European Society of Haematology, Lisbon, 1963 (ESH 1964) and the 10th Congress of the International Society of Haematology (ISH), Stockholm, 1964 (ISH 1965). The International Committee for Standardization in Haematology (ICSH) made recommendations endorsed by the General Assembly of ICSH in Sydney on 23 August 1966 (ICSH 1967), for a reference method for haemoglobinometry and for the manufacture and distribution of an international reference preparation. Further symposia were held at the 12th Congress of the ISH, New York, 1968 (Astaldi, Sirtori&Vanzetti 1979) and at the 13th Congress of ISH, Munich, 1970 (Izak&Lewis 1972). The recommendations were reissued in 1978 (ISH 1978). On the basis of continuing experimental studies, the reference method and the specifications for the international reference preparation have been modified. The revised recommendations are described in this document.
