Summary.Osteonecrosis is usually considered to be a minor and rare late sequelae of treatment for Hodgkin's Disease (HD). However, over ten years six patients out of a total of 53 patients treated for HD at the haematology department of a district general hospital developed osteonecrosis, an incidence of 11.3%. Routine isotope bone scanning of patients produced two further asymptomatic cases. These results suggest a higher incidence than previously suspected of a problem which caused our patients considerable morbidity. Further measures are required to identify and adequately treat a significant cause of late morbidity following chemotherapy for HD.

Summary.The significance of precipitating causes of acute disseminated intra‐vascular coagulation (DIC) and the severity of derangement of haemostasis based on laboratory investigations carried out initially were evaluated in 98 patients and was related to the fatal outcome in them. It was seen that septicaemia was the commonest precipating cause. Survival was better in patients in whom DIC was precipitated by obstetric causes compared with those with septicaemia (P2.5) as compared to their lower values (P1.5. Summary.Haemophilia B is due to multiple molecular defects in the factor IX gene. Most of them are single base substitutions, and can now be identified by direct sequencing of the coding sequence of the factor IX gene, preceded or not by a screening strategy. In some instances the mutation alters an enzyme recognition site and this allows rapid and accurate carrier testing and prenatal diagnosis in the affected pedigree. This was not the case for the previously described nt 31119 (G–>A) mutation that we found in an extended haemophilia B pedigree, during the search for mutations in the factor IX gene in patients from Southern France. We first detected this mutation by single stranded conformation polymorphism (SSCP) and then identified it by DNA sequencing. Carriership could be easily determined in the females of the pedigree by analysis of the SSCP patterns. Summary.We report the data of direct and indirect immunofluorescence labelling of peripheral blood mononuclear cells obtained from 40 normal controls and nine patients having blood tests for non‐haematological disorders (PNHD controls) using flow cytometric analysis. Polyclonal and monoclonal antibodies were used to define the expression of cell surface antigen of T cells, their subsets, B cells, natural killer cells (NK) and myeloid cells. Normal values of absolute number and percentages of each of the populations of mononuclear cells were established and were sufficiently reproducible to be of clinical use. The percentages of positive values of T cells, T‐cell subsets, and B cells in ten normal controls when mononuclear cells were used, were comparable to those using whole blood lysis, APAAP immuno‐alkaline phosphatase, and E and M rosettes. The results obtained were similar in both the normal and PNHD controls. In most instances the percentage recovery of identified T, B and NK cells was complete. Summary.The Coulter® STKS was evaluated in a children's hospital, in order to (a) compare theWBCdifferential given by the instrument to a 400 cell visual differential (reference method); (b) evaluate the sensitivity and specificity of the alarm system, and (c) provide data concerning the use and interpretation of results in children. 653 blood samples were collected. The Coulter® STKS results were studied in 523 patients having no morphological abnormalities in the blood smears, separated into subgroups according to the presence of STKS alarms and according to age. The results were found accurate both in STKS negative and STKS positive patients (i.e., those with alarms:‘Blasts', Imm Gran 2, Variant Lymph, NRBC, review slide). Negative STKS results had the same accuracy in all age groups, except in neonates where slide review must be systematically performed. The instrument exhibited a good sensitivity of the suspect flags studied (91.4%), with a lower specificity (72%) reflecting the number of false positive results found in our group, probably due to the cytological features particular to children. Summary.We have studied an alternative method to aggregometry for the assessment of human polymorphonuclear (PMN) leucocyte aggregation. This simple, rapid and reliable procedure counts unaggregated cells on both Coulter’STKS and STKR haematological analysers by the impedance principle. Aggregation of PMN was induced by 15 min incubation with fresh autologous serum (FAS) after a 10 min phorbol myristate acetate (PMA) activation of neutrophils in small aliquots (0.25 ml) of suspension containing about 4.0 × 109PMN/l. Differences (x 100) between count of resting and PMA + FAS treated neutrophils/count of resting PMN reflect percent aggregation. By this procedure, PMN aggregation did not occur in autologous plasma from EDTA anticoagulated whole blood; it was partially inhibited by hydrocortisone, whereas inactivated or Zymosan activated sera gave values similar to those from FAS induced aggregation. PMA aggregation was dependent on Ca2++Mg2+concentration. Intra‐assay analytical variability did not exceed 4% on either instrument. Reference values (n= 20) of percent PMN aggregation were 50.7 ± 4.7 on STKS and 47.1 ± 4.8 on STKR. Most probably, the interindividual variance was due to the physiological variability of Mg2+and/or Ca2+concentrations in FAS. Summary.The Laboratory Proficiency Testing Program tests and evaluates proficiency of clinical laboratories in Ontario, Canada. The programme began in 1974; testing of reticulocyte counts was added in 1982. From the beginning, the goal of the programme has been to ensure that the quality of practice in Ontario laboratories facilitates good quality patient care. Testing of indirect counting methods for reticulocytes is accomplished by the distribution of stained blood films. Performance is expressed in terms of accuracy and precision. Proficiency is assessed by comparing submitted results to an all‐methods’mean and result reliability by observing the coefficient of variation (CV) of the cohort. The quality of the results vary neither with the manual counting system used nor the type of laboratory. The CV commonly ranges from 25 to 30%, and this lack of accuracy appears to be inherent in the technology of manual reticulocyte counting. Summary.A highly automated laboratory data processing environment for use with Coulter STKS haematology analysers is described. This in‐house system is based on a network of personal computers running Novell Network software and communicating with an IBM® mainframe. The system captures the leucocyte scatterplot graphic and numeric parameters from the blood analysers, integrating this with patient demographic and request data held on the host computer. This composite picture is then reviewed on the network by laboratory personnel before generating printed reports.
