摘要:
Much controversy exists today regarding the“salvaging”of high-risk infants. There are many ethical concerns surrounding these children. The advancement of technology has had a profound effect on the treatment of preterm infants or infants with deformities. In this article the authors address selected ethical concerns regarding personhood, autonomy, beneficence, parental authority, quality of life, sanctity of life, and optional, obligatory, and wrong forms of treatment.The article begins with an overview of the history of infanticide as well as the history of neonatal intensive care units. Financial and legal issues are also discussed. In addition, the nurse's role in working with high-risk infants and their families is briefly explored.

Cystic Fibrosis (CF) is a disease that has profound consequences on a family unit. As a genetically transmitted, chronic illness, it influences every aspect of family life, including time constraints, finances, and relationships among family members. The purpose of this article is to demonstrate the cumulative effect of the nursing diagnoses related to the pathophysiology of CF on the nursing diagnosis, Potential for Alterations in Family Process, when a family has a child with cystic fibrosis.This nursing diagnosis was first recognized by the North American Nursing Diagnosis Association (NANDA) in 1982. It is vital that nurses be able to differentiate between a family having difficulty coping with a child with CF in the home, and one that is coping well on its own. The children of families that are not coping well tend to be hospitalized more than those of families that are coping well. The families use the hospitalizations as rest periods for themselves. By recognizing the cues identified by NANDA of a family having problems, early intervention can be initiated before the family is in a crisis. The family must live with CF on a daily basis for the remainder of the child's life; therefore it must be able to function at its optimum level. It is nursing's responsibility to be able to recognize and assist the family in need. A knowledge of the nursing diagnosis, Alterations in Family Process, will assist nurses in fulfilling this obligation to the family

摘要:
Otitis media with effusion (OME) is a common disease of childhood that has a profound influence on family life. The findings of a grounded theory study that explored family interaction when a child has OME are contained in this article. Three variables were found to influence the position of the family on a continuum of learning to manage: effects of the disease on the child, disruption to family life, and relationship with the health care system. In this discussion the authors focus on the effects on the child and family life. Comparisons are made with the effects of chronic illness on a family. The documentation of the effects of OME on family life is significant to assist health care professionals in appreciating the gty of this often trivialized illness. Some strategies are suggested for positive health promotion in these families.

As increased numbers of teen mothers choose to keep and rear their children, the topic of their childrearing abilities has become the subject of intense theorizing, philosophizing, and research. Research about the developmental deviations of teen mothers' children has suggested that teen mothers are at risk for childrearing failure. The mechanisms through which the multiple, complex, and interactive variables operate to produce the children's less than optimal developmental outcomes remain obscure. In this article selected examples of teen childrearing research are reviewed to provide the background for a discussion of selected methodological and conceptual issues. Implications for nursing practice and research are discussed in terms of a collaborative researcher-clinician effort.

Feeling lonely is a sad but relatively common experience for children, adolescents, and adults. When loneliness becomes a frequently occurring phenomenon, there are serious implications for emotional and physical well-being. The concept of loneliness can be described in relation to psychosocial development.Loneliness in children and adolescents is a relatively new research topic. In this paper the author reviews research for both age groups.Pediatric nurses should recognize the importance of identifying children and adolescents at high risk for the development of chronic loneliness. Nursing interventions for assisting the lonely child or adolescent are described briefly.
