This article contains an intensive and extensive summary of reasons why, how, and for what purpose free clinics came into existence. The initial discussion centers around what the unmet needs of the community are that helped to promote the coming into existence of free clinics. The discussion then goes into the problems and the planning necessary for setting up the administrative machinery for the clinic, and issues of the lay board vs. the steering committee. There is a presentation of volunteerism, how to keep these men and women in an alternative institution, and the role of the steering committee. The procedure for how to admit a patient, the appropriate record keeping, and the maintenance of quality care is essential in this article. The concept of service for service as a method of fee payment is raised. The Director takes us through his clinic as if we were a woman patient who comes in for assistance. A medical history chart is filled out by a woman intake person and the patient is referred to the Triage person, the bridge between the patient, the medical section, and the laboratory. Further material deals with very basic questions that any free clinic should ask itself before it opens: What are the health problems in this community? Who are its community leaders and how can we contact them? What are the health laws? And what are the pharmaceutical regulations in the city and the state? There is discussion of malpractice insurance and tax status. The importance of counselor sensitization is brought out, as well as the importance of training. A very thorough and detailed presentation of the organization and the materials required in the examining room, the pharmacy, and the laboratory is given, as well as the procedures for the sterilization of nondisposable instruments, and how to obtain medical and dental supplies.

Molica and Winn present the philosophy of a free clinic. They describe setting up of an advisory board, the miscalculations that they made in the type of clients they thought they would be seeing, and the fact that they did not foresee that they would not receive cooperation from the local medical facilities and counseling centers. The article goes into the struggles that ensued among the board and the staff, struggles that almost lead to closing down the Waikiki clinic. There is then a discussion of reorganization, the definition of the Executive Director's job functions, the opening of a halfway house, and the consequent good utilization of the community.

This article presents a philosophical and practical look at the free clinic movement from a third world people's perspective. The free clinic is seen as a significant alternative to the lack of health care in the black communities. Serious questions are raised about setting up discrete clinics to meet the needs of the community in a haphazard way. In terms of personal qualities, a competent free clinic counselor ought to be able to manifest a degree of warmth and humor, feel comfortable with people, and have a facility and liking for people. The chapter also discusses techniques of staff training, the need for common objectives, the use of role playing, films, tapes, and video‐tape equipment, the possibility of using two‐counselor teams both for training and for the possibility of one's substituting for the other in case of absence. Techniques and “how to” in terms of crisis counseling for individual and group work are presented. The crisis intervention approach is demonstrated, as well as means of assessing strengths and weaknesses in those who seek help. Issues of the dynamics of the counseling staff are discussed, the subtle dependency relationships that are established, the problems of transference and mutual need that may be acted upon through counseling. The gay movement has brought about increasing awareness of the need for gay counseling by gay men and women. This chapter presents the group effort of gay people and their manner of getting their clinic together.
