The histopathologic findings in the temporal bones of two patients who underwent a cryo-otic-perotic shunt procedure are reported. Morphologic changes in the inner ear were limited to the area of cryoprobe application on the promontory. The hook region of the cochlear duct showed severe atrophy of the spiral ligament, stria vascularis, and sensorineural structures. The ampulla of the posterior semicircular canal showed fibrosis in the perilymphatic space and ruptured membranous walls.No changes were found in the rest of the vestibular labyrinth or in the facial nerve. The long-term improvement in these patients' symptoms, i.e., eight years in one and four years in the other, could only be explained on the basis that endolymph and perilymph mix across intact, nonfunctioning membranes of the cochlear duct or through, presumably antemortem, ruptures of the posterior semicircular canal ampulla.

摘要:
We argue that the scope of basic audiometric assessment must be broadened to include speech audiometric measures sensitive to central auditory dysfunction. Findings in twenty cases of retrocochlear disorder illustrate the fact that conventional criteria of audiometric normalcy are inadequate.

Tympanometric results were reviewed for 341 cases of Meniere's disease and 200 cases of sensorineural hearing loss of unknown causes. The results indicate that the incidence of Eustachian tube dysfunction in patients with Meniere's disease is nearly the same as in a control group of patients with sensorineural hearing loss of unknown causes: The percentage of patients with Meniere's disease having Eustachian tube dysfunction was similar to the percentage of normal-hearing listeners with Eustachian tube dysfunction reported by Jerger.

A Caucasian male suffered a skull fracture at age twenty-two. There was bilateral deafness with partial recovery of hearing in the left ear. He was able successfully to wear a hearing aid in this ear for only thirteen years. His balance remained impaired until death at age fifty of unrelated causesTemporal bone histologic studies revealed bilateral transverse fractures extending through the ampullated end of the three semicircular canals and vestibula. The cochleae were not involved. In the right ear, the organ of Corti was missing in the basal 14 mm and shrunken in the remainder of the cochlea. In the left ear, the organ of Corti was missing in the basal 14 mm and present with partial hair cell loss in the remainder of the cochlea. Spiral ganglion neuron loss correlated in severity with the organ of Corti lesions bilaterally. The membranous vestibular labyrinth showed distension and ruptures with total loss of sensory epithelium of the cristae and maculae bilaterallyProgressive deterioration of left ear residual hearing cannot be explained on the basis of sensorineural deficit; it probably was the result of biochemical alterations. Total loss of vestibular sensory epithelium bilaterally explained the persistent disequilibrium.

Meningiomas rarely arise from or remain limited to the temporal bone. When the diagnosis of temporal bone meningioma is made, it is mandatory to investigate the possibility of an intracranial component. Computerized axial tomography with contrast enhancement is of limited value in detecting small tumors adjacent to the temporal bone or within the internal auditory canal. If the computerized tomogram is normal, a positive contrast posterior fossa myelogram is indicated. The pitfall of depending solely upon computerized tomography to detect intracranial extension became clear in the evaluation of a patient found to have an unusual trilobed meningioma.

Adam Politzer wrote this article on the history of mastoid surgery for inclusion in Volume II of his History of Otology, Enke Verlag, Stuttgart, 1913.Politzer reviews the decades before antibiotics when the simple or complete mastoidectomy was performed in cases of acute mastoiditis in which abscess formation was suspected; such timely surgery after prevented extensive osseous destruction and also often-fatal meningitis and sinus thrombosis.So-called “radical” surgery began when surgeons realized that simple exposure of the antrum was inadequate to control chronic infection involving the middle ear, ossicles, and meatal walls. “Modified radical” surgery evolved from the attempt to preserve hearing, canal plasty from the attempt to prevent postoperative canal stricture. Politzer describes the surgery designed to close the tympanic ostium of the Eustachian tube and the application of skin grafts to the operative sites.
