AbstractAnesthesia of the caudal margin of the nose and the upper lip is of considerable importance to otolaryngologists. It follows, therefore, that the sensory innervation is also important. In situations where adequate anesthesia is needed and limited quantity must be used, it is advantageous to know where that limited amount could be best placed to accomplish the greatest anesthesia.Due to transmedian innervation of the upper lip by the infraorbital nerves, it may be necessary to block both sides when procedures are to be carried out in the vicinity of the midline. The infraorbital nerves of each side cross the area of the frontonasal process and ultimately decussate in the midline. No other nerves were found to participate in sensory innervation of the upper lip.

AbstractThe charts of 85 patients who had undergone an approach to the anterior cervical spine between 1966 to 1970 were reviewed. Nine of them (11 percent) showed postoperative voice changes, three of those had permanent vocal cord paralysis. The possible causes of recurrent nerve damage are discussed.

AbstractThe management of mandibular fractures in edentulous patients poses special problems of reduction and immobilization of the bone fragments. Intramedullary pinning as a means of internal fixation is a direct and simple procedure which permits the free movement of the jaw immediately after the operation.The present study supports the conclusion that intramedullary pinning may be considered the procedure of choice for the treatment of fractures of the body and most fractures of the angle of the edentulous mandible.

AbstractThe treatment of patients with advanced carcinoma of the base of the tongue is difficult. Selected patients with this lesion can be aided by an aggressive therapeutic approach to this problem. Combined therapy consisting of irradiation therapy followed by composite resection of the entire tongue and larynx and neck dissection may offer the best opportunity for cure or palliation in these patients. AbstractThe diagnosis of disseminated coccidioidomycosis was made in two pregnant women by the biopsy of facial granulomas. The unusual presentation and the problems associated with the management of this disease in pregnancy stimulated a literature review and a report of these two cases.The history of the discovery and elucidation of this disease is reviewed. Documentation of interhuman transmission of coccidioidomycosis by fomites is presented. Previous reports of disseminated coccidioidomycosis being found in the endolarynx, epiglottis and region of the head and neck are cited. The high incidence of fetal wastage, the high maternal mortality associated with dissemination and the role of Amphotericin B in the therapy of the disseminated disease are discussed.

AbstractSurgery in the area of the larynx often demands identification of laryngeal motor nerves. Stimulation of the recurrent laryngeal nerve caused observable movement of the mobilized larynx in 56 exposures on the right side and 52 exposures on the left, except in two cases. Failure of the larynx to move in two cases was explained by fixation of the larynx due to cancer in one case and fixation due to scar in the other. The use of the operating microscope and electrical stimulator is recommended as an improvement in the technique for the identification of the laryngeal motor nerves. A color motion picture with sound is presented in conjunction with this paper to demonstrate the technique.

AbstractThe scanning electron microscope is an exciting instrument which is easily adapted by the clinician‐researcher to the study of tissues of otolaryngologic interest. The SEM is based on the television raster principle of scanning a surface with a thin beam of electrons. As a result of the interaction of the electron beam with the surface, a number of physical phenomena are elicited including the emission of secondary electrons. AbstractThe literature of cystic hygroma of the neck has been presented. The important aspects of the embryology, clinical picture and treatment have been discussed. Because of the very low incidence of this tumor, most surgeons are unable to gain much personal experience with this intriguing lesion. Consideration of the reports of others and a retrospective review of his own cases has led the author to establish and suggest certain guidelines in dealing with this tumor.
